观察盐酸莫西沙星片联合四联抗结核方案对结核性胸膜炎的治疗效果

Observation on therapeutic effect of moxifloxacin hydrochloride tablets combined with quadruple anti-tuberculosis regimen on tuberculous pleurisy

目的观察盐酸莫西沙星片联合四联抗结核方案治疗结核性胸膜炎患者的临床效果。方法60例结核性胸膜炎患者,应用掷硬币法分为参照组与观察组,每组30例。参照组患者采用四联抗结核治疗方案,观察组患者则在参照组基础上增加盐酸莫西沙星片治疗。比较两组治疗前后T淋巴细胞亚群水平、免疫能力和炎性因子水平、不良反应发生情况。结果治疗后,两组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平均较治疗前升高,且实验组高于参照组,差异有统计学意义(P<0.05)。治疗后,实验组肿瘤坏死因子-α(TNF-α)(10.54±1.21)ng/L低于参照组的(18.57±2.11)ng/L,γ-干扰素(IFN-γ)(73.58±6.75)ng/L高于参照组的(61.45±6.97)ng/L,差异有统计学意义(P<0.05)。实验组不良反应发生率为20.00%,与参照组的16.67%比较,差异无统计学意义(P>0.05)。结论盐酸莫西沙星片联合四联抗结核方案治疗结核性胸膜炎,能够有效改善T淋巴细胞水平,增强机体免疫能力,降低炎症反应,且用药安全,具有临床推广价值。

Objective To observe the clinical effect of moxifloxacin hydrochloride tablets combined with quadruple anti-tuberculosis regimen on tuberculous pleurisy.Methods A total of 60 patients with tuberculous pleurisy were divided into reference group and observation group by coin tossing method,with 30 cases in each group.The reference group was treated with quadruple anti-tuberculosis regimen,and the observation group was treated with moxifloxacin hydrochloride tablets based on the reference group.The T lymphocyte subsets,immune function and inflammatory factors before and after treatment,and occurrence of adverse reactions were compared between the two groups.Results After treatment,the CD3,CD4^(+)and CD4^(+)/CD8^(+)of the two groups were higher than those before treatment,and the experimental group was higher than the reference group;all the difference was statistically significant(P<0.05).After treatment,the tumor necrosis factor-α(TNF-α)(10.54±1.21)ng/L of the experimental group was lower than(18.57±2.11)ng/L of the reference group,and interferon-γ(IFN-γ)(73.58±6.75)ng/L was higher than(61.45±6.97)ng/L of the reference group.All the difference was statistically significant(P<0.05).The incidence of adverse reactions of the experimental group was 20.00%,which had no statistically significant difference compared with 16.67%of the reference group(P>0.05).Conclusion Combination of moxifloxacin hydrochloride tablets and quadruple anti-tuberculosis regimen can effectively improve the T lymphocyte of patients with tuberculous pleurisy,enhance the immune ability,reduce the inflammatory reaction.It is safe to use,and has the value of clinical promotion.
