抗心力衰竭对充血性心力衰竭患者的疗效及对BNP和CA125表达的影响

Clinical effects of anti-heart failure treatment in treatment of heart failure and its effects on BNP and CA125 expression

目的探讨心力衰竭治疗对充血性心力衰竭患者的利钠肽(BNP)、糖类抗原125(CA125)表达的影响及临床意义。方法2016年3月至2017年3月心内科收诊的充血性心力衰竭患者120例,按随机分表法分为对照组和观察组,每组60例。对照组遵医嘱给予常规治疗,观察组给予常规治疗+抗心力衰竭治疗。观察2组治疗后的血浆中BNP、CA125、心功能分级变化、心功能指标等的影响及临床疗效。结果2组患者治疗2周后、4周后、6周后的BNP浓度及CA125A水平均下降,且观察组优于对照组(P<0.05);2组患者的各项心功能指标LVEDD、LVEF、6MWT的水平等均有所改善(P<0.05);同时观察组患者的LVEDD、LVEF、6MWT的指标变化均优于对照组(P<0.05);观察组患者心功能分级的改善状况均优于对照组(P<0.05)。结论常规抗心力衰竭治疗能改善患者的心功能分级及临床症状,显著降低充血性心力衰竭患者血清中的BNP及CA125的水平;监测血浆中BNP水平及血清中CA125水平也可协助医师筛查、诊断及治疗,同时还可判断患者治疗后的预后,值得广泛应用,并通过不断实践完善BNP、CA125等在临床诊断中的应用。

Objective To investigate the effects of anti-heart failure treatment on the expressions of BNP and CA125 in patients with congestive heart failure and its clinical significance.Methods A total of 120 patients with congestive heart failure who were treated in our hospital from March 2016 to March 2017 were enrolled in the study,who were randomly divided into control group and observation group,with 60 cases in each group.The patients in control group were given routine treatment according to doctor’s advice,however,the patients in observation group,on the basis of control group,were given anti-heart failure treatment.After treatment,the changes of plasma BNP,CA125,heart function grading,cardiac function and clinical efficacy were observed and compared between the two groups.Results The levels of BNP and CA125A at 2w,4w,6w after treatment in both groups were decreased,moreover,which in observation group were superior to those in control group(P<0.05).The cardiac function indexes including LVEDD,LVEF,and 6MWT in both groups were significantly improved(P<0.05),moreover,which in observation group were superior to those in control group(P<0.05).In addition the improvement status of heart function in observation group was superior to that in control group(P<0.05).Conclusion The conventional anti-heart failure treatment can improve the heart function classification and clinical symptoms of patients with congestive heart failure,and can significantly reduce the serum levels of BNP and CA125,moreover,to monitor the plasma BNP levels and serum CA125 levels can assist physicians in screening,diagnosis and treatment,evaluating the prognosis of patients,therefor,which is worth using widely in clinical practice.
