RA患者血清RF、MMP-3及抗CarP抗体联合检测的诊断价值

Diagnostic value of combined detection of serum RF,MMP-3 and anti carp antibody in RA patients

目的探讨对类风湿性关节炎(RA)患者行血清类风湿因子(RF)、基质金属蛋白酶(MMP-3)及抗氨基甲酰化(CarP)抗体联合检测的诊断价值。方法以我院2018年1月~2019年12月期间收治的RA患者94例作为观察组,非RA良性免疫性疾病患者92例作为对照组,以及健康体检者96例作为健康组。对比分析三组人群的RF、MMP-3及抗CarP抗体水平以及分析其相关性。采用Logistic回归分析进行多因素分析,并联合检测RF、MMP-3及抗CarP抗体水平判断RA的诊断效能。结果观察组患者的血清RF、MMP-3及抗CarP抗体水平均高于对照组和健康组(P<0.05),对照组高于健康组(P<0.05)。三组血清RF、MMP-3及抗CarP抗体水平间呈的正相关性(P<0.05)。血清RF、MMP-3及抗CarP抗体水平均是RA疾病的独立危险因素。除特异度稍低于MMP-3指标检测外,联合检测的敏感度、特异度、约登指数及ROC曲线下面积均高于独立检测。结论RF、MMP-3及抗CarP抗体联合检测在RA疾病诊断中具有较高敏感性和特异性,具有较好的临床应用价值。

Objective To discuss the diagnostic value of combined detection of RF,MMP-3 and anti-carp antibody in patients with rheumatoid arthritis(RA).Methods 94 patients with RA admitted to our hospital from January 2018 to December 2019 were selected as the observation group,92 patients with non RA benign immune diseases were selected as the control group,and 96 healthy people were selected as the healthy group.The levels and correlation of RF,MMP-3 and anti carp antibody in the three groups were compared and analyzed.The relationship between the levels of RF,MMP-3 and anti carp antibody was analyzed by logistic regression analysis.The diagnostic efficacy of RA was evaluated by joint detection of RF,MMP-3 and anti carp antibody levels.Results The serum RF,MMP-3 and anti carp antibody levels in the observation group were significantly higher than those in the control group and the healthy group(P<0.05),and the control group was higher than the healthy group(P<0.05).There was a significant positive correlation between the levels of serum RF,MMP-3 and anti carp antibody among the three groups(P<0.05).Logistic regression analysis showed that serum RF,MMP-3 and anti carp antibody levels were independent risk factors of RA.The sensitivity,specificity,yoden index and area under ROC curve of combined detection were higher than those of MMP-3 respectively.Conclusion The combined detection of RF,MMP-3 and anti carp antibody has high sensitivity and specificity in the diagnosis of RA disease.
