62例骨髓增生异常综合征FAB和WHO分型及形态学特点 被引量:1

摘要 目的分析骨髓增生异常综合征(MDS)的FAB和世界卫生组织(WHO)标准的分型特点,了解其病态造血特点,对MDS的分型及细胞形态学有进一步的认识。方法对中山大学附属第三医院2017年至2021年确诊的62例MDS患者(男33例,女29例,中位年龄为62岁)标本,分别按FAB和WHO标准分型后进行细胞形态学观察,分析粒系、红系及巨核系的病态造血情况。结果通过对62例MDS标本的细胞形态学分析,得出MDS的重要病态造血特征:粒系为颗粒减少或缺失、Pelger-Huet畸形,红系为奇数核及核间桥,巨核系为淋巴样小巨核细胞。WHO分型更符合实际情况的需要。结论FAB及WHO分型标准对MDS病态造血特征的观察有重要的指导意义。 Objective To analyze the typing characteristics of FAB and world health organization(WHO)criteria in myelodysplastic syndrome(MDS),to understand its pathological hematopoietic characteristics,and to have a further understanding on the classification and morphocytology of MDS.Methods The morphocytology observation on 62 MDS patients in The Third Affiliated Hospital of Sun Yat-sen University from 2017 to 2021 was carried out after FAB and WHO typing,including 33 males and 29 females,with a median age of 62 years old,and the pathological hematopoiesis in the granule system cells,erythroid system cells,and trophic nucleus system cells were analyzed.Results Through the morphological analysis of 62 MDS specimens,the important pathological hematopoietic characteristics of MDS were obtained:particle reduction or absence and Pelger-Huet malformation in the granule system cells,nuclear budding or the bridge between nuclear in the erythroid system cells,and lymphoid micromegakaryocyte in the trophic nucleus system cells.WHO typing was more in line with the actual situation.Conclusion FAB and WHO typing criteria have important guiding significance for the observation on pathological hematopoietic characteristics of MDS.
