关节镜辅助下内固定关节融合术治疗晚期踝关节炎患者的临床研究

Clinical Study of internal fixed joint Fusion under arthroscopic Assist in the Treatment of advanced Ankle Arthritis

目的探讨关节镜辅助下内固定关节融合术治疗晚期踝关节炎患者的应用效果。方法回顾性分析2016年5月至2017年5月浚县人民医院收治的86例晚期踝关节炎患者临床资料,依据手术方案不同分为两组,各43例。对照组予以开放性踝关节融合术,研究组予以关节镜辅助下内固定关节融合术,对比两组手术指标(术中出血量、手术时间、关节融合时间)、术后24个月踝―后足功能优良率、术后并发症发生情况。结果研究组术中出血量低于对照组,手术时间、关节融合时间均短于对照组(P<0.05);术后24个月,研究组踝―后足功能优良率高于对照组(P<0.05);研究组术后并发症发生率低于对照组(P<0.05)。结论晚期踝关节炎患者采用关节镜辅助下内固定关节融合术治疗可减少术中出血量,有效缩短手术时间及关节融合时间,减少术后并发症发生率,远期疗效良好。

Objective To investigate the effect of internal fixed joint fusion under arthroscopic assist in the treatment of advanced ankle arthritis.Methods A retrospective analysis of the clinical data of 86 patients with advanced ankle arthritis admitted to xun County People’s Hospital from May 2016 to May 2017 was divided into two groups according to different surgical procedures,with 43 cases in each group.The control group was treated with open ankle joint fusion,and the study group was treated with internal fixed joint fusion under arthroscopic assist,the operative indexes(intraoperative bleeding,operative time,joint fusion time),excellent and good rate of ankle posterior foot function and postoperative complications were compared between the two groups.Results Compared with the control group,the blood loss in the study group was lower,the operation time and the joint fusion time were shorter(P<0.05),and the excellent and good rate of ankle posterior foot function in the study group was higher(P<0.05)at 24 months after operation;The incidence of postoperative complications in the study group was lower than that in the control group(P<0.05).Conclusion Internal fixed joint fusion under arthroscopic assist can reduce the amount of bleeding,shorten the time of operation and joint fusion,reduce the rate of postoperative complications,and have a good long-term effect.
