自拟咳喘宁方治疗支气管哮喘急性发作对患者呼吸功能和氧化应激水平的影响 被引量:5

Effect of Self-designed Kechuanning Decoction on Patients′ Respiratory Function and Oxidative Stress Level in the Treatment of Acute Bronchial Asthma Attacks

摘要 目的观察自拟咳喘宁方治疗支气管哮喘急性发作对患者呼吸功能和氧化应激水平的影响。方法选取我院于2019年1月至2020年4月收治的支气管哮喘急性发作患者114例,根据治疗方案分为两组。对照组根据《支气管哮喘急性发作评估及处理中国专家共识》拟定治疗方案,观察组加用自拟咳喘宁方治疗。统计两组疗效和不良反应,收集两组呼吸功能指标、肺功能指标及氧化应激因子水平。结果观察组总有效率为92.98%,高于对照组的77.19%(P<0.05)。治疗后,观察组和对照组的呼气流速峰值(PEFR)、最大自主通气量(MVV)、第1秒用力呼气量占预计值百分比(FEV1%)、FEV1/用力肺活量(FVC)上升,气道阻力(Raw)下降,且观察组PEFR、MVV、FEV1%、FEV1/FVC较对照组高,Raw较对照组低(均P<0.05)。治疗后,观察组和对照组的总抗氧化能力(T-AOC)上升,活性氧(ROS)、丙二醛(MDA)、8-羟基脱氧鸟苷(8-OHdG)下降,且观察组T-AOC较对照组高,ROS、MDA、8-OHdG较对照组低(均P<0.05)。两组不良反应发生率差异无统计学意义(P> 0.05)。结论自拟咳喘宁方治疗支气管哮喘急性发作可减少氧化应激损伤,改善肺功能和呼吸功能,提高疗效。 Objective:To observe the effect of self-desinged Kechuanning Decoction on patients′respiratory function and oxidative stress level in the treatment of acute bronchial asthma.Methods:A total of 114 patients with acute bronchial asthma admitted to our hospital from January 2019 to April 2020 were selected and divided into two groups according to the treatment plan.The control group developed a treatment plan according to the“Chinese Expert Consensus on the Assessment and Management of Acute Bronchial Asthma”,and the observation group was treated with self-designed Kechuanning Decoction.The efficacy and adverse reactions of the two groups were statistically analyzed,and the respiratory function indexes,lung function indexes and oxidative stress factor levels of the two groups were collected.Results:The total effective rate of the observation group was higher than that of the control group with statistical significance(P<0.05).After treatment,the peak expiratory flow rate(PEFR),maximum voluntary ventilation(MVV),FEV1%,and FEV1/FVC increased in the observation group and the control group.Air way resistance(RAW)decreased,and PEFR,MVV,FEV1%,FEV1/FVC in the observation group were higher than those in the control group,and RAW was lower than the control group(P<0.05).After treatment,the total antioxidant capacity(T-AOC)of the observation group and the control group increased,while reactive oxygen species(ROS),malondialdehyde(MDA),8-hydroxydeoxyguanosine(8-OHdG)decreased,and the T-AOC in the observation group was higher than that of the control group,ROS,MDA,8-OHdG were lower than those of the control group(P<0.05).The incidence of adverse reactions between the two groups was not statistically significant(P>0.05).Conclusion:Self-designed Kechuanning Decoction can reduce the oxidative stress damage,improve the lung function and respiratory function,and improve the curative effect of acute bronchial asthma.
