基于护理人员的老年患者肠镜检查前肠道准备效果不佳的质性研究

Qualitative study of poor bowel preparation before enteroscopy in elderly patients based on nursing staff

目的了解消化内镜中心护理人员在老年患者肠镜检查前肠道准备中发挥作用不佳原因,为今后实施针对性的护理干预和护理管理提供参考。方法采用质性研究的现象学方法和目的抽样对山西省4所三级甲等综合医院内镜中心的16名护理人员进行半结构访谈,运用Colaizzi资料分析原则对资料进行整理分析。结果通过半结构访谈,护理人员在肠镜检查前肠道准备中宣教不到位是老年患者肠道准备效果不佳的因素之一,其次,老年患者身体机能的下降,伴随其理解力及记忆力的减退,往往忽视了肠道准备的重要性,对肠道准备知识理解不足等也导致肠道准备效果不佳。结论临床中,医务人员应加强对老年患者肠镜检查的宣教,引导其正确理解肠道准备过程并认识肠道准备的重要性,同时发挥家属在老年患者肠道准备中的正性作用。其次,积极探索更为便捷的肠道准备方式,针对不同的人群结合信息化的手段,探索更为积极有效的肠道准备宣教方式,以减轻临床护理工作负担及任务。

Objective To understand the reasons for the poor role of nursing staff in digestive endoscopy center in the intestinal preparation before enteroscopy in elderly patients, and to provide a reference for the implementation of targeted nursing intervention and nursing management in the future.Methods The phenomenological method of qualitative research was used to sample the semi-structured interviews of 11 nursing staff in a tertiary hospital in Shanxi Province.The data were collated and analyzed using Colaizzi data analysis principles.Results Through semi-structured interviews, inadequate education of nursing staff during bowel preparation before colonoscopy was one of the factors that caused poor bowel preparation in elderly patients.Second, the decline in physical function of elderly patients was accompanied by their understanding and memory decline of the intestinal preparation often ignores the importance of intestinal preparation, and the lack of understanding of intestinal preparation knowledge also leads to poor intestinal preparation.Conclusion In clinical practice, medical personnel should strengthen the education of colonoscopy in elderly patients, guide them to correctly understand the intestinal preparation process and recognize the importance of intestinal preparation, and at the same time play the positive role of family members in intestinal preparation of elderly patients.Secondly, actively explore more convenient intestinal preparation methods, and combine information-based methods for different groups to explore more active and effective intestinal preparation education methods to reduce the burden and tasks of clinical nursing.
