摘要
目的:分析河南中医药大学第一附属医院(以下简称“我院”)2018—2019年抗菌药物使用情况,探讨促进抗菌药物合理应用的管理措施。方法:采用回顾性分析方法,随机抽取我院2018年、2019年各1000例住院患者,并对抗菌药物的使用情况、销售金额、用药频度(DDDs)、序号比和合理用药情况进行分析。结果:与2018年住院患者相比,我院2019年住院患者抗菌药物使用率有所降低,单用、二联使用和限制使用级抗菌药使用构成比较前增加(P<0.05);喹诺酮类、青霉素类、氨基糖苷类等抗菌药物的DDDs较前升高;我院使用的抗菌药物中序号比小于1的较多;抗菌药物使用不合理各类问题中,适应证不适宜、药品遴选不适宜、用法用量不适宜等问题的构成比较高。结论:我院抗菌药物临床应用中仍存在一些用药不合理问题,可通过药学干预进一步加强抗菌药物管理,促进抗菌药物合理应用工作持续改进。
Objective:To analyze the use of antibacterial drugs in our hospital in 2018 and 2019,and to explore management measures to improve the rational use of antibacterial drugs.Methods:The retrospective analysis method was used.Hospitalized patients in our hospital in 2018 and 2019 were randomly selected,each year 1000 cases.The use condition,sales amount,defined daily dose system(DDDs),serial number ratio and rational drug use of antibacterial drugs were analyzed.Results:Compared with that in 2018,the use rate of antibacterial drugs in hospitalized patients of our hospital decreased in 2019,and the constituent ratio of single medication,duplex medication and restricted-use antibacterial drugs increased(P<0.05).DDDs of quinolones,penicillins,aminoglycosides increased.Many antibacterial drugs used in our hospital with the serial number ratio less than 1.Among the irrational use of antibacterial drugs,the constituent ratio of inappropriate indication for drug use,inappropriate drug selection,inappropriate usage and dosage were relatively high.Conclusion:Irrational drug use still exist in the clinical application of antibacterial drugs in our hospital,and the management of antibacterial drugs can be further strengthened through pharmaceutical intervention,so as to promote continuous improvement of the rational application of antibacterial drugs.
