安氏Ⅲ类错[牙合]畸形患者前方牵引前行反复扩缩的效果研究

Research on effect of repeated expansion and contraction before anterior traction in patients with class Ⅲ malocclusion

目的探讨安氏Ⅲ类错[牙合]畸形患者前方牵引前行反复扩缩的效果。方法采用前瞻性随机试验方法,选择2019年6月—2020年10月在赣州市立医院进行治疗的80例安氏Ⅲ类错[牙合]畸形患儿作为研究对象,随机编号1~80号,将单数归为对照组,双数归为观察组,各40例。对照组给予前方牵引治疗,观察组在对照组基础上于前方牵引前行反复扩缩,于治疗5个月结束后对比2组头影测量指标。结果治疗5个月结束时,2组蝶鞍点-鼻根点-上齿槽座点角(SNA)、上齿槽坐点-鼻根点-下齿槽座点角(ANB)、上中切牙长轴与前颅底平面交角(U1-SN)、下颌平面与前颅底平面交角(MP-SN)及Wits值均较治疗前高,且观察组SNA、ANB及MP-SN高于对照组,U1-SN低于对照组,差异有统计学意义(P<0.05)。结论前方牵引配合反复扩缩治疗安氏Ⅲ类错[牙合]畸形效果确切,可增强患者上颌骨位置改善效果。

Objective To investigate the effect of repeated expansion and contraction before anterior traction in patients with Angle ClassⅢmalocclusion.Methods A prospective randomized trial method was used to select 80 children with Angle Class Ⅲ malocclusion treated in Ganzhou Municipal Hospital from June 2019 to October 2020 as the research subjects,randomly number them 1-80,the odd number was divided into the control group and the even number into the observation group,40 cases in each group.The control group was given the treatment of anterior traction,and the observation group was given repeated expansion and contraction before anterior traction on the basis of the control group,after 5 months of treatment,the cephalometric indexes of the two groups were compared.Results At the end of 5 months of treatment,the angle of sella-nasion-A point(SNA),A point-nasion-B point(ANB),the angle between the long axis of upper central incisor and the anterior skull base plane(U1-SN),the angle of between mandibular plane and the anterior skull base plane(MP-SN)and the Wits value(vertical line from the upper and lower alveolar seat to the functional occlusal plane,the distance between the two perpendicular feet)were all higher than those before treatment,the SNA,ANB and MP-SN of the observation group were higher than those of the control group,and U1-SN was lower than that of the control group,with statistical significant differences(P<0.05).Conclusion The effect of anterior traction combined with repeated expansion and contraction in the treatment of Angle Class Ⅲ malocclusion is definite,which can enhance the improvement effect of maxillary position.
