视网膜血流状态指导白内障手术安全性的临床研究

Clinical study on the safety of cataract surgery guided by retinal blood flow state

目的探讨视网膜血流状态指导白内障手术安全性的临床效果。方法将化州市人民医院2019年9月—2020年9月诊治的单纯性白内障患者32例(Ⅰ组)、慢性病引起的白内障患者32例(Ⅱ组)、正常眼健康体检人员32例(Ⅲ组)作为研究对象,3组人员均进行彩色多普勒眼部视网膜血流超声检查,对比视网膜血流状态。结果在3组人员眼动脉血流中,Ⅰ组及Ⅱ组的舒张末期血流速度(Vd)明显低于Ⅲ组(P<0.05)。Ⅰ组的Vd高于Ⅱ组,但无统计学差异(P>0.05)。Ⅰ组、Ⅱ组及Ⅲ组的血管阻力指(RI)、收缩期血流速度(Vs)比较,差异无统计学意义(P>0.05)。在3组人员睫状后动脉血流中,Vs、Vd及RI比较,差异无统计学意义。在3组人员视网膜中央动脉血流中,RI比较Ⅰ组<Ⅲ组<Ⅱ组,差异有统计学意义(P<0.05)。Ⅰ组及Ⅱ组的Vd低于Ⅲ组,差异有统计学意义(P<0.05)。Ⅰ组及Ⅱ组的Vd相比较,差异无统计学意义(P>0.05)。Ⅰ组、Ⅱ组及Ⅲ组的Vs相比较,差异无统计学意义(P>0.05)。结论视网膜血流状态与白内障有一定的关系,与是否是由慢性病引起白内障关系不大,可为指导白内障手术提供重要的临床参考依据。

Objective To explore the clinical effect of retinal blood flow state in guiding the safety of cataract surgery.Methods Thirty-two patients with simple cataract(Group Ⅰ),32 patients with chronic diseases(GroupⅡ)and 32 healthy people(Group Ⅲ)treated in our hospital from September 2019 to September 2020 were taken as research objects.Before phacoemulsification,all the three groups were examined by color Doppler ultrasound.The retinal blood flow status of the three groups was compared.Results In the ophthalmic artery blood flow of the three groups,the end diastolic blood flow velocity(Vd)of group Ⅰ and group Ⅱ was significantly lower than that of group Ⅲ(P<0.05).The Vd of group Ⅰ was higher than that of group Ⅱ,but the comparison between groups was p>0.05.There was no significant difference in vascular resistance index(RI)and systolic blood flow velocity(Vs)among group Ⅰ,group Ⅱ and group Ⅲ(P>0.05).In the blood flow of posteRIor ciliary artery among the three groups,there was no significant difference in Vs,Vd and ri among the three groups(P>0.05).RI in the central retinal artery blood flow of the three groups was significantly different(P<0.05).The Vd of group Ⅰ and group Ⅱ was lower than that of group Ⅲ(P<0.05).There was no significant difference in Vd between group Ⅰ and groupII(P>0.05).Comparing the Vs of group Ⅰ,group Ⅱ and group Ⅲ,there was no significant difference(P>0.05).Conclusion The retinal blood flow state has a certain relationship with cataract,but has little relationship with whether cataract is caused by chronic diseases.This blood flow state provides an important clinical reference for guiding the safety of cataract surgery.
