甘露醇注射液联合丁苯酞注射液对急性脑梗死患者治疗效果、氧化应激反应的影响 被引量:13

摘要 目的观察甘露醇注射液联合丁苯酞注射液对急性脑梗死患者临床效果及氧化应激反应的影响。方法选取2017年7月—2018年7月在本院接受治疗的急性脑梗死78例,根据治疗方法的不同分为观察组和对照组各39例。对照组给予常规治疗,观察组在对照组基础上给予甘露醇注射液联合丁苯酞注射液治疗,2组均连续治疗15 d。比较2组治疗前后丙二醛(MDA)、超氧化物歧化酶(SOD)、氧化型低密度脂蛋白(ox-LDL)、谷胱甘肽过氧化物酶(GSH-Px)水平,治疗前后基底动脉、左大脑中动脉、右大脑中动脉平均血流速度,神经功能、生活质量改善情况,治疗后临床疗效,以及不良反应发生情况。结果2组治疗后MDA、ox-LDL水平及美国国立卫生研究院卒中量表(NIHSS)评分低于治疗前,SOD、GSH-Px水平及Barthel指数评定量表评分高于治疗前,差异有统计学意义(P<0.05);治疗后观察组MDA、ox-LDL水平及NIHSS评分低于对照组,SOD、GSH-Px水平及Barthel指数评定量表评分高于对照组,差异有统计学意义(P<0.05,P<0.01)。2组治疗后基底动脉、左大脑中动脉、右大脑中动脉平均血流速度快于治疗前(P<0.05);治疗后观察组基底动脉、左大脑中动脉、右大脑中动脉平均血流速度快于对照组(P<0.01)。观察组治疗总有效率高于对照组(P<0.05)。2组不良反应总发生率比较差异无统计学意义(P>0.05)。结论甘露醇注射液联合丁苯酞注射液能减轻急性脑梗死患者氧化应激反应程度,促进患者神经功能恢复,提高患者生活质量和治疗效果,且安全性较好。 Objective To observe clinical efficacy of Mannitol Injection combined with Butylphthalide Injection in treatment of patients with acute cerebral infarction(ACI)and its effect on oxidative stress reaction.Methods A total of 78 patients with ACI treated between July 2017 and July 2018 were selected and divided into observation group(n=39)and control group(n=39)according to different treatment methods.Control group was given conventional treatment,while observation group was given Mannitol Injection combined with Butylphthalide Injection on the basis of treatment for control group.Both groups were treated for 15 d consecutively.Levels of malondialdehyde(MDA),superoxide dismutase(SOD),oxidized low density lipoprotein(ox-LDL),glutathione peroxidase(GSH-Px),mean blood flow velocity values of basilar artery,left middle cerebral artery and right middle cerebral artery,neurological function,quality of life before and after treatment,clinical efficacy and incidence rate of adverse reactions were compared between two groups.Results After treatment,levels of MDA,ox-LDL and the National Institutes of Health Stroke Scale(NIHSS)scores were significantly lower,while levels of SOD and GSH-Px and scores of Barthel index rating scale were significantly higher than those before treatment in two groups(P<0.05).After treatment,in observation group,levels of MDA and ox-LDL and NIHSS score were significantly lower,while levels of SOD and GSH-Px and score of Barthel index rating scale were significantly higher than those in control group(P<0.05,P<0.01).After treatment,mean blood flow velocity values of basilar artery,left middle cerebral artery and right middle cerebral artery were faster than that before treatment in two groups(P<0.05);mean blood flow velocity values of basilar artery,left middle cerebral artery and right middle cerebral artery in observation group were faster than those in control group(P<0.01).The total effective rate in observation group was higher than that in control group(P<0.05).There was no significant difference in the total incidence rate of adverse reactions between two groups(P>0.05).Conclusion Mannitol Injection combined with Butylphthalide Injection may reduce the degree of oxidative stress reaction,promote the recovery of neurological function and improve the quality of life and clinical efficacy with good safety in patients with ACI.
