摘要 目的探讨微RNA-16(microRNA-16,miR-16)及Let-7a在支气管哮喘患者中表达水平及与严重程度的关系。方法选取2017年7月至2019年7月我院收治的128例支气管哮喘患者作为研究对象,按照严重程度将患者分为轻、中度组(n=85)和重度组(n=43)。选取同期120例体检健康者作为对照组。采用实时荧光定量PCR(RT-qPCR)检测血清miR-16和Let-7a相对表达量,并分析模型A(miR-16+Let-7a)、模型B(IL-13+IgE+EOS计数)及模型C(miR-16+Let-7a+IL-13+IgE+EOS计数)评估重度支气管哮喘的效能。结果轻、中度组及重度组血清miR-16和Let-7a相对表达量低于对照组,重度组血清miR-16和Let-7a相对表达量低于轻、中度组,差异均有统计学意义(P<0.05)。miR-16、Let-7a评估重度支气管哮喘的AUC分别为0.802、0.808。Logistic回归分析显示IL-13、IgE、EOS计数、miR-16、Let-7a与支气管哮喘病情加重密切相关。模型C评估重度支气管哮喘的效能高于模型A及模型B。结论血清miR-16和Let-7a与支气管哮喘严重程度关系密切,检测血清miR-16和Let-7a相对表达量有助于评估患者病情。 Objective To investigate the expression level of microRNA-16(miR-16)and let-7a in patients with asthma and their relationship with severity.Methods A total of 128 patients with bronchial asthma admitted to our hospital from July 2017 to July 2019 were selected as the subjects.The patients were divided into the light and moderate groups(n=85)and the severe group(n=43)according to severity.120 healthy people in the same period were selected as the control group.Real-time quantitative PCR(RT-qPCR)was used to detect the relative expression levels of miR-16 and Let-7a,and the model A(miR-16+Let-7a),the model B(IL-13+IgE+EOS count)and the model C(miR-16+Let-7a+IL-13+IgE+EOS count)were used to evaluate the efficacy of severe bronchial asthma.Results The relative expression of serum miR-16 and Let-7a in the light and moderate group and the severe group were lower than those in the control group,and the relative expressions of serum miR-16 and Let-7a in the severe group were lower than those in the light and moderate group(P<0.05).The AUC of miR-16 and Let-7a in assessing severe bronchial asthma were 0.802 and 0.808,respectively.Logistic regression analysis showed that IL-13,IgE,EOS count,miR-16,and Let-7a were closely related to the severity of bronchial asthma.Model C was more effective than Model A and B in evaluating severe bronchial asthma.Conclusion Serum miR-16 and Let-7a are closely related to the severity of bronchial asthma.The detection of relative expression of serum miR-16 and Let-7a is helpful to evaluate the patient's condition.
