偏头痛风火候患者脑突显网络功能分析 被引量:1

Analysis of salience network function in migraine patients with feng-huo syndrome

摘要 目的观察并分析偏头痛风火候患者脑突显网络(SN)功能改变。方法选择20例无先兆偏头痛风火候青年患者纳入偏头痛组,以年龄、性别匹配的20例青年健康志愿者纳入对照组,进行静息态功能磁共振(rs-fMRI)扫描。用GIFT3.0b软件对两组rs-fMRI数据进行独立成分分析确定SN,然后采用REST1.8软件分析两组SN差异脑区并提取差异脑区Reho值,分析Reho值与正念注意觉知量表(MAAS)、焦虑自评量表(SAS)、抑郁自评量表(SDS)评分的相关性。结果偏头痛组左侧小脑后叶、左侧梭状回、左侧尾状核头、右侧前扣带回、左侧额中回、右侧背外侧前额叶皮层脑功能较对照组降低,偏头痛组右侧舌回脑功能较对照组活跃。右侧背外侧前额叶皮层Reho值与SDS呈负相关(r=-0.535,P=0.015)。结论偏头痛风火候患者SN在左小脑后叶、左侧梭状回、左侧尾状核头、右侧前扣带回、左侧额中回、右侧背外侧前额叶皮层功能下降,右侧舌回功能增强,其中右侧背外侧前额叶皮层功能与SDS呈负相关。 Objective To observe and analyze the functional changes of the brain salience network(SN)in patients with migraine feng-huo syndrome.Methods Twenty migraine without aura patients with feng-huo syndrome(migraine group)and 20 matched(age and gender)healthy controls(control group)were selected to undergo resting-state function⁃al magnetic resonance imaging(rs-fMRI).We used GIFT3.0b software to perform independent component analysis on the two sets of rs-fMRI data to determine the SN,and then used REST1.8 software to analyze the SN in different brain areas of the two groups and obtained the Reho values.We calculated the correlation between the Reho values and the Mindful At⁃tention Awareness Scale(MAAS),Self-Rating Anxiety Scale(SAS),and Self-Rating Depression Scale(SDS).Results The migraine group had decreased function in the left posterior cerbellar lobe,left fusiform gyrus,left head of caudate nu⁃cleus,right anterior cingulate,left middle frontal gyrus,and right dorsolateral prefrontal cortex,and increased function in the right lingual gyrus as compared with the control group.The Reho value in the right dorsolateral prefrontal cortex was negatively correlated with the SDS(r=-0.535,P=0.015).Conclusion Patients in the migraine group showed neuro⁃function decrease in the left posterior cerebellum,right fusiform gyrus,left caudate nucleus head,right anterior cingulate gyrus,left middle frontal gyrus,and left dorsal prefrontal cortex,but function enhancement in the right lingual gyrus;the Reho value in the left and dorsal prefrontal cortex was negatively correlated with the SDS score.
