西药联合清肺宽胸理气汤治疗慢性阻塞性肺疾病加重期57例疗效观察

Effect of Western Medicine Combined with Qingfei Kuanxiongliqi Decoction on 57 Cases of Exacerbation of Chronic Obstructive Pulmonary Disease

目的:观察西药联合清肺宽胸理气汤治疗慢性阻塞性肺疾病加重期的疗效。方法:采用前瞻性随机试验方法,选择慢阻肺急性加重期患者113例,采用掷硬币法分为对照组(n=56)和观察组(n=57)。在常规治疗基础上,对照组给予西药治疗,在对照组基础上,观察组联合中药清肺宽胸理气汤治疗。对比两组治疗2周后的疗效、血气分析指标[动脉血氧分压(PO(2))、动脉血二氧化碳分压(PCO(2))]和呼吸力学指标[吸气峰值压(PIP)、平均气道压(MPaw)、气道阻力(Raw)]。结果:观察组临床总有效率(92.98%)较对照组(76.79%)高,差异有统计学意义(P<0.05);治疗后,两组PaO(2)水平比治疗前高,PaCO(2)、PIP、MPaw、Raw水平均低于治疗前,且观察组更低,差异有统计学意义(P<0.05)。结论:慢性阻塞性肺疾病急性加重期患者采用西药联合清肺宽胸理气汤治疗,可改善血气分析以及呼吸力学相关指标,疗效明显。

Objective To observe the effect of Western medicine combined with Qingfei Kuanxiong Liqi Decoction in the treatment of exacerbation of chronic obstructive pulmonary disease.Methods 113 patients with acute exacerbation of COPD in our hospital from January 2019 to December 2019 were selected and divided into control group(n=56)and observation group(n=57)by coin tossing method.On the basis of conventional treatment,the control group was treated with western medicine,on the basis of the control group,the observation group was treated with Qingfei Kuanxiong Liqi decoction.The therapeutic effect,blood gas analysis indexes[arterial partial pressure of oxygen(PO(2)),arterial partial pressure of carbon dioxide(PCO(2))]and respiratory mechanics indexes[peak inspiratory pressure(PIP),mean airway pressure(mPaw),airway resistance]were compared between the two groups after 2 weeks of treatment.Results the total effective rate of the observation group(92.98%)was higher than that of the control group(76.79%),the difference was statistically significant(P<0.05);After treatment,the level of PaO(2)in the two groups was higher than that before treatment,the levels of PaCO(2),Pip,mPaw and raw in the two groups were lower than those before treatment,and the difference was statistically significant(P<0.05).Conclusion western medicine combined with Qingfei Kuanxiong Liqi Decoction in the treatment of acute exacerbation of chronic obstructive pulmonary disease can improve blood gas analysis and respiratory mechanics related indicators,with obvious curative effect.
