摘要
目的:通过与MRI神经导航进行对比分析,评价超声-MRI融合导航技术在神经外科手术中的应用价值。方法:纳入2018年1月—2018年12月在我院接受神经外科手术患者,分为试验组(超声-MRI融合导航组,34例)和对照组(MRI神经导航组,62例),所有患者均经病理证实。对比分析2组患者一般资料(性别、年龄、病灶大小、病灶深度、术后病理)有无差异;术前,对脑漂移距离与病灶深度进行相关性分析;以术后CT为参照,比较超声-MRI融合导航和MRI神经导航的手术全切除率。结果:①2组患者性别、年龄、病灶大小、病灶深度、病理构成方面的差异均无统计学意义(P>0.05)。②随着病灶深度的增加,脑漂移距离减小,两者呈现强负相关性(r=-0.918,P<0.05)。③术后CT显示,试验组病灶全切除率、对所有病灶切缘判断准确率均高于对照组,差异有统计学意义(P<0.05);其中对良性病灶切缘判断准确率在2组间差异无统计学意义(P>0.05),而对恶性病灶切缘判断准确率,试验组明显高于对照组,两者差异有统计学意义(P<0.05)。结论:超声-MRI融合导航技术能实时纠正脑漂移,提高恶性病灶的全切除率。
Purpose:By comparing with MRI neuronavigation,we evaluate the application value of ultrasound-MRI fusion navigation technology in neurosurgery operation.Methods:From January 2018 to December 2018,34 patients in the study group(ultrasound-MRI fusion navigation group)and 62 patients in the control group(MRI navigation group)were enrolled in this study.All patients were confirmed by pathology results.The distribution of general data(gender,age,size of mass,depth and pathology result)in the two groups were compared and analyzed.The correlation between brain shift distance and the depth of the mass was analyzed.The total removal rate of ultrasound-MRI fusion navigation and MRI neuronavigation was compared with that of CT after operation.Results:①There was no significant difference in sex,age,size,depth and pathological composition between the two groups(P>0.05).②With the increasing of the depth of the lesion,the distance of brain drift decreased.There was a strong negative correlation between them,and the correlation coefficient was-0.918.③Postoperative CT showed that the total resection rate of lesions and the accuracy rate of judging the cutting edge of all lesions in the experimental group were higher than those in the control group(P<0.05);There was no significant difference between the two groups in the accuracy rate of benign lesion margin judgment(P>0.05),but the accuracy rate of malignant lesion margin judgment in the experimental group was significantly higher than that in the control group(P<0.05).Conclusion:Ultrasound-MRI fusion navigation technology can rectify brain drift in real time and improve the total resection rate of malignant tumors.
