小切口非超声乳化囊外摘除术治疗老年白内障的效果和对视力的影响

Effect of small incision non-phacoemulsification extracapsular extraction in the treatment of senile cataract and its influence on vision

目的观察小切口非超声乳化囊外摘除术治疗老年白内障的效果及对视力的影响。方法选取2018年2月—2020年4月北京市仁和医院眼科收治的76例老年白内障患者作为研究对象,按照随机数字表法将其分为常规组(38例)与研究组(38例),常规组患者采用超声乳化白内障吸除术方法,研究组患者采用小切口非超声乳化囊外摘除术方法。比较两组患者的眼压、视力以及不良反应发生情况。结果两组治疗前的眼压比较,差异无统计学意义(P>0.05);常规组治疗后的眼压高于治疗前,差异有统计学意义(P<0.05);研究组治疗后的眼压高于治疗前,差异有统计学意义(P<0.05);研究组的治疗后2、6、12及24h眼压均低于常规组,差异有统计学意义(P<0.05)。两组治疗前及治疗后3个月的视力比较,差异无统计学意义(P>0.05);常规组治疗后的视力高于治疗前,差异有统计学意义(P<0.05);研究组治疗后的视力高于治疗前,差异有统计学意义(P<0.05);研究组的治疗后1周视力高于常规组,差异有统计学意义(P<0.05)。研究组的不良反应总发生率低于常规组,差异有统计学意义(P<0.05)。结论老年白内障患者在选择治疗方法上,可以将小切口非超声乳化囊外摘除术考虑在内,安全有效,值得应用。

Objective To observe the effect of small incision non-phacoemulsification extracapsular excision in the treatment of senile cataract and its influence on vision.Methods A total of 76 elderly cataract patients who were admitted to the Department of Ophthalmology of Renhe Hospital of Beijing from February 2018 to April 2020 were selected as the research objects,and they were divided into a conventional group(38 cases)and a study group(38 cases)according to the random number table method.Patients in the conventional group used phacoemulsification and extracapsular phacoemulsification patients in the study group used small incision non-phacoemulsification extracapsular extraction.The intraocular pressure,visual acuity and adverse reactions of patients of the two groups were compared.Results The comparison of the intraocular pressure before treatment between the two groups was not statistically significant(P>0.05);the intraocular pressure after treatment in the conventional group was higher than that before treatment,and the difference was statistically significant(P<0.05);the intraocular pressure after treatment in the study group was higher than that before treatment,the difference was statistically significant(P<0.05);the intraocular pressure of the study group at 2,6,12 and 24h after treatment was lower than that of the conventional group,and the difference was statistically significant(P<0.05).The visual acuity comparison between the two groups before treatment and 3 months after treatment was not statistically significant(P>0.05);the visual acuity after treatment in the conventional group was higher than that before treatment,and the difference was statistically significant(P<0.05);The visual acuity after treatment in the study group was higher than that before treatment,and the difference was statistically significant(P<0.05);the visual acuity of the study group was higher than that of the conventional group at 1 week after treatment,and the difference was statistically significant(P<0.05).The total incidence of adverse reactions in the study group was lower than that in the routine group,and the difference was statistically significant(P<0.05).Conclusion In the choice of treatment for elderly cataract patients,small-incision non-phacoemulsification extracapsular extirpation can be taken into consideration,which is safe and effective and worthy of application.
