摘要
目的研究急性ST段抬高型心肌梗死(STEMI)患者血清缺氧诱导因子-1α(HIF-1α)含量与病情及经皮冠状动脉介入术(PCI)后院内不良心血管事件(MACE)的关系。方法选择2018年7月至2020年7月期间在本院接受急诊PCI治疗的急性STEMI患者160例作为STEMI组、同期体检的健康志愿者110例作为对照组,检测血清HIF-1α含量;评估STEMI患者的病情及院内MACE,采用logistic回归模型分析MACE的影响因素,采用ROC曲线分析MACE的预测因素。结果 STEMI组患者的血清HIF-1α含量高于对照组,差异有统计学意义(P<0.05),STEMI组中HIF-1α<中位数患者的LDH、CK-MB、cTnI、Gensini评分、Grace评分、院内MACE发生率高于HIF-1α≥中位数患者,LVEF低于HIF-1α≥中位数患者,差异有统计学意义(P<0.05);LVEF、Grace评分、HIF-1α是院内MACE的影响因素(P<0.05);血清HIF-1α含量是预测院内MACE的指标(P<0.05)。结论急性STEMI患者血清HIF-1α含量与病情加重及PCI后发生MACE有关,对院内MACE具有预测价值。
Objective To investigate the relationship between serum hypoxia inducible factor-1α(HIF-1α)level and the disease condition,major adverse cardiovascular events(MACE)after percutaneous coronary intervention(PCI) of patients with acute ST segment elevation myocardial infarction(STEMI).Methods Acute STEMI patients who received emergency PCI treatment in our hospital from July 2018 to July2020 were selected as the STEMI group,and healthy volunteers who received physical examination at the same time were selected as the control group. The serum HIF-1α level was detected. The disease condition and MACE in hospital of STEMI patients were evaluated. The influencing factors of MACE was analyzed by logistic regression model,and the predictive factors of MACE were analyzed by ROC curve. Results The level of serum HIF-1α in the STEMI group was higher than that in the control group,and the difference was statistically significant(P<0.05). In the STEMI group,the incidence of LDH,CK-MB,cTnI,Gensini score,Grace score,and MACE in patients with HIF-1α< median was higher than that of patients with HIF-1α≥ median,and LVEF was lower than HIF-1α ≥ median number of patients,the difference was statistically significant(P<0.05).LVEF,Grace score,HIF-1α are the influencing factors of MACE in the hospital(P<0.05). The level of serum HIF-1α is an indicator for predicting MACE in the hospital(P<0.05). Conclusion The level of serum HIF-1αin patients with acute STEMI is related to the aggravation of the disease and the occurrence of MACE after PCI,which has predictive value for MACE in the hospital.
