动态心电图对原发性高血压患者心肌缺血及心律失常诊断价值分析

Analysis of Diagnostic Value of Dynamic Electrocardiogram for Myocardial Ischemia and Arrhythmia in Patients with Essential Hypertension

目的分析动态心电图对原发性高血压患者心肌缺血及心律失常诊断价值。方法选择2019年2月—2020年2月该院收治原发性高血压92例患者,按随机数表分为两组,对照组46例使用常规心电图检查,研究组46例采用动态心电图检查,对两组心肌缺血与心律失常检出率进行比较;对比两组ST-T改变与房室传导阻滞的检出率。结果研究组心肌缺血、室上性心律失常与室性心律失常检出率分别为86.96%、89.13%、86.96%,比对照组高,差异有统计学意义(χ^(2)=4.998、18.178、7.014,P<0.05);研究组ST-T改变、I°传导阻滞、Ⅱ°I型传导阻滞及Ⅱ°Ⅱ型传导阻滞检出率73.91%、69.57%、60.87%及39.13%比对照组高,差异有统计学意义(χ^(2)=8.788,4.483,4.348,4.246,P<0.05)。结论临床将动态心电图检查应用于原发性高血压患者,可提高其心肌缺血及心律失常检出率,能为临床早诊断及早治疗提供有效依据。

Objective To analyze the value of Holter in the diagnosis of myocardial ischemia and arrhythmia in patients with essential hypertension.Methods 92 patients with primary hypertension admitted to the hospital from February 2019 to February 2020 were selected and divided into two groups according to the random number table.46 cases in the control group were examined by conventional electrocardiogram,and 46 cases in the study group were examined by dynamic electrocardiogram checked.Compared the detection rates of myocardial ischemia and arrhythmia between the two groups;compared the detection rates of ST-T changes and atrioventricular block in the two groups.Results The detection rates of myocardial ischemia,supraventricular arrhythmia and ventricular arrhythmia in the study group were 86.96%,89.13%,and 86.96%,respectively,which were higher than those of the control group.The difference was statistically significant(χ^(2)=4.998,18.178,7.014,P<0.05).The detection rates of ST-T changes,I°block,Ⅱ°I block andⅡ°Ⅱblock in the study group were 73.91%,69.57%,60.87%and 39.13%higher than those in the control group,the difference was statistically significant(χ^(2)=8.788,4.483,4.348,4.246,P<0.05).Conclusion The clinical application of Holter examination in patients with essential hypertension can increase the detection rate of myocardial ischemia and arrhythmia,and can provide an effective basis for clinical early diagnosis and early treatment.
