经皮椎间孔脊柱内镜技术治疗继发性腰椎管狭窄症的临床研究

Clinical Study of Percutaneous Foraminal Spinal Endoscopy in the Treatment of Secondary Lumbar Spinal Stenosis

目的探究经皮椎间孔脊柱内镜技术治疗继发性腰椎管狭窄症(LSS)的临床疗效。方法于2018年10月—2019年10月选取60例继发性LSS患者,随机分为参照组(n=30)、研究组(n=30),分别予以传统开放手术、经皮椎间孔脊柱内镜技术治疗,观察手术情况、Osweatry功能障碍指数(ODI)、临床疗效。结果研究组手术时间、术中出血量、住院时间、术后VAS评分均低于参照组,差异有统计学意义(t=8.578、31.952、10.209、6.966,P<0.05);研究组术后ODI低于参照组,差异有统计学意义(t=8.111,P<0.05);研究组治疗总有效率93.33%略高于参照组的80.00%,差异无统计学意义(χ^(2)=2.308,P=0.129)。结论以经皮椎间孔脊柱内镜技术治疗继发性LSS,能够在保证临床治疗效果同时,降低手术创伤、缩短住院时间、减轻疼痛及功能障碍,具有推广应用价值。

Objective To explore the clinical efficacy of percutaneous foraminal spinal endoscopy in the treatment of secondary lumbar spinal stenosis(LSS).Methods From October 2018 to October 2019,60 patients with secondary LSS were selected and randomly divided into the reference group(n=30)and the study group(n=30).They were given traditional open surgery and percutaneous intervertebral foraminal spine endoscopic technology treatment,observation of the operation status,Osweatry dysfunction index(ODI),and clinical efficacy.Results The operation time,intraoperative blood loss,hospitalization time,and postoperative VAS score of the study group were lower than those of the reference group,the difference was statistically significant(t=8.578,31.952,10.209,6.966,P<0.05);the postoperative ODI of the study group was lower than the reference group,the difference in the reference group was statistically significant(t=8.111,P<0.05);the total effective rate of treatment in the study group was 93.33%slightly higher than the reference group's 80.00%,the difference was not statistically significant(χ^(2)=2.308,P=0.129).Conclusion The treatment of secondary LSS with percutaneous interforaminal spine endoscopy can reduce surgical trauma,shorten hospitalization time,and relieve pain and dysfunction while ensuring the clinical treatment effect.It has the value of popularization and application.
