摘要
目的探讨行宫内复苏术后,对新生儿神经功能和足月胎儿窘迫脐血指标的影响情况。方法选择2017年3月—2019年3月来该院就诊的产妇共132例,依照计算机表法分组,各66例。将实施常规治疗的产妇作为参照组,将实施宫内复苏术治疗的产妇作为研究组。检验两组新生儿的神经功能指标以及脐血指标。结果研究组产妇的PaO2、pH值、HCO3-相比于参照组较高,研究组产妇的PaCO2相比于参照组较低,组间数据对比差异有统计学意义(P<0.05);出生后1、3、5、7 d研究组产妇的S-100蛋白、神经元特异性烯醇化酶(neuron-specific enolase,NSE)水平明显低于参照组,组间数据差异有统计学意义(P<0.05)。研究组出生后7 d产妇的S-100蛋白水平为(1.02±0.13)ng/L、NSE水平为(11.85±1.32)ng/L。参照组出生后7 d产妇的S-100蛋白水平为(2.46±1.24)ng/L、NSE水平为(26.57±3.32)ng/L,组间数据差异有统计学意义(t=9.383、33.471,P<0.05)。结论对产妇实施宫内复苏术治疗,足月胎儿窘迫情况得到明显改善,将胎儿酸中毒、宫内缺氧缺血情况得到缓解,其新生儿神经功能受到保护。
Objective To investigate the effects of intrauterine resuscitation on neonatal nerve function and cord blood indicators of full-term fetal distress.Methods From March 2017 to March 2019,a total of 132 cases of parturients who came to the hospital for treatment were included,and were grouped according to the computer table method,with 66 cases in each case.The parturients undergoing conventional treatment were regarded as the reference group,and the parturients undergoing intrauterine resuscitation were regarded as the study group.Test the neurological function indexes and cord blood indexes of the two groups of newborns.Results The PaO2,pH,and HCO3-of the study group were higher than those of the reference group,and the PaCO2 of the study group was lower than that of the reference group,the difference was statistically significant(P<0.05);maternal levels of S-100 protein and NSE(neuron-specific enolase)in the study group at 1 d,3 d,5 d,and 7 d after birth were significantly lower than the reference group,the data difference between the groups was statistically significant(P<0.05).The S-100 protein level of the pregnant women in the study group was(1.02±0.13)ng/L and the neuron-specific enolase(NSE)level was(11.85±1.32)ng/L 7 d after birth.The S-100 protein level of the parturients in the reference group was(2.46±1.24)ng/L and the NSE level was(26.57±3.32)ng/L 7 d after birth in the reference group,the difference was statistically significant(t=9.383,33.471,P<0.05).Conclusion The implementation of intrauterine resuscitation for parturients can significantly improve the distress of the full-term fetus,relieve fetal acidosis,intrauterine hypoxia and ischemia,and protect the neonatal neurological function.
