摘要
目的探讨雌二醇(E2)、C反应蛋白(CRP)、纤维蛋白原(FIB)、胱抑素C(Cys C)的关系。方法根据《中国绝经管理与绝经激素治疗指南2018》中围绝经期诊断标准,及《中国高血压防治指南2018》高血压诊断标准:选取100名为围绝经期高血压组,100名为围绝经期非高血压组,采集并记录所有入组人员的一般资料年龄、吸烟史、饮酒史、BMI及病史情况,测量记录血压,完善包括E2、CRP、FIB、Cys C、FPG、TC、HDL-C、LDL-C、TG等生化指标。结果围绝经期高血压组与围绝经期非高血压组相比,一般临床资料比较中年龄、吸烟史、饮酒史、BMI、FPG、TC、HDL-C、LDL-C、TG比较均无统计学意义(P>0.05)。而E2水平较低,差异显著(P<0.01),在围绝经期高血压组(n=100),E2分别与CRP(r=-0.512)、FIB(r=-0.211)、Cys C(r=-0.278)呈显著负相关,且均具有统计学意义(P<0.05)。logistic回归分析:E2(OR=0.859,P<0.01)、FIB(OR=2.506,P<0.05)与围绝经期女性高血压独立相关。结论(1)E2水平的下降、CRP、FIB、Cys C水平的上升在围绝经期高血压的发生、发展和维持中起到重要作用。(2)围绝经期女性E2水平的下降可造成CRP、FIB、Cys C水平升高,从而促进围绝经期女性高血压的发生。(3)E2的变化及炎症反应、凝血系统功能异常等共同参与了围绝经期女性高血压的发生发展过程。(4)E2是围绝经期女性高血压的保护因素,FIB是围绝经期女性高血压的危险因素。
Objective The purpose is to explore the relationship between estradiol(E2),C-reactive protein(CRP),fibrinogen(FIB),and cystatin C(Cys C).Methods According to the“Chinese Menopause Management and Menopausal Hormone Treatment Guidelines 2018”the diagnostic criteria of perimenopause,and the“Chinese Hypertension Prevention and Control Guide 2018”hypertension diagnostic criteria:100 patients in the perimenopausal hypertension group and 100 patients in the perimenopausal hypertension group were selected In the non-hypertensive group,collect and record the general information about age,smoking history,drinking history,BMI,and medical history of all participants,measure and record blood pressure,including E2,CRP,FIB,Cys C,FPG,TC,HDL-C,LDL-C,TG and other biochemical indicators.Results The perimenopausal hypertension group compared with the perimenopausal non-hypertensive group,the general clinical data comparison of age,smoking history,drinking history,BMI,FPG,TC,HDL-C,LDL-C,and TG were not statistically significant(P>0.05).The level of E2 was low and the difference was significant(P<0.01).In the perimenopausal hypertension group(n=100),E2 was compared with CRP(r=-0.512),FIB(r=-0.211),Cys C(r=-0.278)showed a significant negative correlation,and both were statistically significant(P<0.05).Logistic regression analysis:E2(OR=0.859,P<0.01),FIB(OR=2.506,P<0.05)were independently associated with hypertension in perimenopausal women.Conclusion(1)The decrease of E2 level and the increase of CRP,FIB and Cys C levels play an important role in the occurrence,development and maintenance of perimenopausal hypertension.(2)The decrease of E2 level in perimenopausal women can increase the levels of CRP,FIB and Cys C,and further promote the occurrence of hypertension in perimenopausal women.(3)The changes of E2,inflammation,and coagulation system dysfunction are jointly involved in the occurrence and development of hypertension in perimenopausal women.(4)E2 is a protective factor for hypertension in perimenopausal women,and FIB is a risk factor for hypertension in perimenopausal women.
