闭合复位外固定和切开复位内固定治疗跟骨骨折的Meta分析

摘要 目的系统评价闭合复位外固定和切开复位内固定治疗跟骨骨折的疗效。方法计算机检索PubMed、Cochrane Library、Embase等数据库。检索时段为建库至2020年12月。筛选文献后评价质量,提取数据,应用RevMan5.3软件进行统计分析。结果纳入5项研究,共376例患者。Meta分析结果:两组在根骨宽度、根骨长度、A OFAS评分及骨折愈合时间方面差异无统计学意义,而闭合复位外固定架组在术中出血量、手术时长及住院时间方面优于切开复位钢板组。结论闭合复位外固定架技术与切开复位钢板内固定技术在治疗跟骨骨折方面有相似固定效果和功能结果,但前者有术中出血量少、手术时间短、住院时间短的优势。 Objective To systematically evaluate the efficacy of closed reduction external fixation and open reduction internal fixation in the treatment of calcaneal fractures.Methods Computer searches PubMed,CochraneLibrary,Embase and other databases.The retrieval period is from the establishment of the database to December 2020.After screening the literature,evaluate the quality,extract the data,and use RevMan5.3 software for statistical analysis.Results Five studies with 376 patients were included.Meta-analysis showed that there was no significant difference in root bone width,root bone length,AOFAS score and fracture healing time between the two groups,but the closed reduction and external fixator group was superior to the open reduction and steel plate group in intraoperative blood loss,operation time and hospitalization time.Conclusion Closed reduction and external fixator technique and open reduction and plate internal fixation technique have similar fixation effects and functional results in the treatment of calcaneal fractures,but the former has the advantages of less intraoperative bleeding,shorter operation time and shorter hospital stay.
