摘要
目的探讨慢性精神分裂症(SCZ)患者血浆白介素17(IL-17)和机体代谢水平的关系。方法方便选择2018年4月—2020年2月入住该院的60例SCZ患者,参照患者血浆IL-17水平的四分位点,将入选病例分为低水平组与高水平组,对比两组一般资料,解读血浆IL-17水平和糖脂代谢之间的关系,并分析影响SCZ患者血浆IL-17水平的主要因素。结果组间患者性别分布、年龄、FPG、TC、TG、LDL-C以及PANSS总分差异无统计学意义(P>0.05);相比之下,低水平组患者发病年龄(11岁)小于高水平组(12岁),HDL-C(1.0±0.2)mmol/L、胰岛素7.5(6.6 mU/L)及胰高血糖素20.5(20.2 ng/L)水平低于高水平组的(1.2±0.4)mmol/L、8.4(9.2 mU/L)、25.9(28.7 ng/L),组间差异有统计学意义(P<0.05);通过Spearman秩相关分析后发现,血浆IL-17水平与HDL-C、胰高血糖素间均存有正相关性(rs=0.180、0.156,P=0.001、0.005<0.05);回归分析后认为,HDL-C[OR=6.25,95%CI(2.30,16.87)]、胰岛素[OR=1.06,95%CI(1.02,1.11)]是影响SCZ患者血浆IL-17水平的主要因素(P<0.05)。结论SCZ患者血浆IL-17高水平时,HDL-C、胰岛素及胰高血糖素水平均有上升,糖脂代谢情况影响着IL-17水平的高低。
Objective To investigate the relationship between plasma interleukin 17(IL-17)and body metabolism in patients with chronic schizophrenia(SCZ).Methods Convenient selected 60 SCZ patients admitted to the hospital from April 2018 to February 2020.According to the quartile of plasma IL-17 levels,the selected cases were divided into low-level and high-level groups,and the general data of the two groups were compared to interpret plasma IL-17 level and glucose and lipid metabolism,and analyzed the main factors affecting the plasma IL-17 level in patients with SCZ.Results There were no statistically significant differences in gender distribution,age,FPG,TC,TG,LDL-C and PANSS total scores between the groups(P>0.05);in contrast,the age of onset(11 years old)of patients in the low-level group was less than the high-level group(12 years old),HDL-C(1.0±0.2)mmol/L,insulin 7.5(6.6 mU/L)and glucagon 20.5(20.2 ng/L)levels were lower than the high-level group(1.2±0.4)mmol/L,8.4(9.2 mU/L),25.9(28.7ng/L),the difference between the groups was statistically significant(P<0.05);after Spearman rank correlation analysis,it was found that the plasma IL-17 level was positive correlated with HDL-C and glucagon(rs=0.180,0.156,P=0.001,0.005<0.05);after regression analysis,it was believed that HDL-C[OR=6.25,95%CI(2.30,16.87)],Insulin[OR=1.06,95%CI(1.02,1.11)]was the main factor affecting the plasma IL-17 level in patients with SCZ(P<0.05).Conclusion When the plasma IL-17 level of SCZ patients is high,the levels of HDL-C,insulin and glucagon all increase,and the metabolism of glucose and lipid affects the level of IL-17.
