综合护理模式在带皮瓣修复口腔癌患者术后组织缺损的效果分析

Analysis of the Effect of Comprehensive Nursing Model in Repairing Postoperative Tissue Defects of Oral Cancer Patients with Skin Flaps

目的探讨综合护理模式在带皮瓣修复口腔癌患者术后组织缺损的效果。方法方便选取2017年6月—2019年12月在该院接受带皮瓣修复手术的口腔癌患者72例,依据患者入院时间先后分作两组,其中对照组36例施以普通护理模式,观察组36例施以综合护理模式。运用统计学软件分析处理两组患者不良反应、皮瓣存活情况及身体各项恢复与心理评估指标。结果观察组患者不良反应的发生率为5.56%,对照组发生率为22.22%,差异有统计学意义(χ^(2)=4.181,P=0.041);观察组皮瓣存活率为97.22%,高于对照组的77.42%,差异有统计学意义(χ^(2)=11.359,P=0.001);观察组患者拔引流管时间为(3.46±0.47)d,术后首次进食时间为(0.36±0.09)d,术后卧床时间为(4.97±0.47)d,术后住院时间为(10.06±1.74)d,短于对照组的(5.41±0.46)、(1.29±0.27)、(7.21±1.47)、(15.05±1.75)d,差异有统计学意义(t=17.791、19.606、8.709、12.132,P<0.05);观察组焦虑状态评分为(27.46±3.24)分,抑郁评分为(29.42±3.04)分,低于对照组,差异有统计学意义(t=17.655、14.267,P<0.05)。结论对带皮瓣修复口腔癌患者术后施以综合护理模式,能够显著降低患者出现不良反应的概率、提高皮瓣存活率和各项恢复指标、使患者心理状态评分保持正常状态,值得在临床护理中进一步推广。

Objective To explore the effect of comprehensive nursing model in repairing postoperative tissue defects of oral cancer patients with skin flaps.Methods Conveniently selected 72 patients with oral cancer underwent flap repair surgery in the hospital from June 2017 to December 2019.They were divided into two groups according to the time they were admitted to the hospital.Among them,36 patients in the control group were treated with ordinary nursing mode,and the observation group 36 cases were given a comprehensive nursing model.Statistical software was used to analyze and process the adverse reactions of the two groups of patients,flap survival,various physical recovery and psychological evaluation indicators.Results The incidence of adverse reactions in the observation group was 5.56%,and the incidence in the control group was 22.22%,the difference was statistically significant(χ^(2)=4.181,P=0.041);the flap survival rate in the observation group was 97.22%,which was higher than the control group 77.42%of patients,the difference was statistically significant(χ^(2)=11.359,P=0.001);the observation group took(3.46±0.47)d to pull out the drainage tube,the first time to eat after the operation was(0.36±0.09)d,and was bedridden after the operation The time was(4.97±0.47)d,and the postoperative hospital stay was(10.06±1.74)d,which was shorter than the control group's(5.41±0.46)d,(1.29±0.27)d,(7.21±1.47)d,(15.05±1.75)d,the difference was statistically significant(t=17.791,19.606,8.709,12.132,P<0.05);the observation group's anxiety state score was(27.46±3.24)points,and the depression score was(29.42±3.04)points,which was lower than control group,the difference was statistically significant(t=17.655,14.267,P<0.05).Conclusion Applying a comprehensive nursing model to patients with oral cancer after repairing oral cancer with skin flaps can significantly reduce the probability of adverse reactions in patients,increase the survival rate of skin flaps and various recovery indicators,and keep patients'mental status scores in a normal state.It is worthwhile further promotion in clinical practice nursing.
