针灸推拿治疗桡骨远端骨折所致肩周炎疗效研究

Acupuncture and Massage Treatment of Periarthritis Caused by Fracture of Distal Radius

目的探讨对桡骨远端骨折所致肩周炎患者采用针灸推拿治疗的临床效果。方法对该院2017年12月—2019年12月收治的桡骨远端骨折的患者展开研究,便利选取其中72例并发肩周炎患者为研究对象,按照双盲法将72例患者分为对照组和研究组,各36例。对照组患者给予中医推拿治疗,研究组患者在此基础上给予针灸治疗。对比两组患者的治疗效果。结果治疗前两组疼痛评分对比,差异无统计学意义(t=0.091,P=0.928);但在治疗1周和2周后,研究组疼痛评分明显低于对照组,差异有统计学意义(t=19.511、24.271,P=0.001、0.001)。治疗前两组中医症候积分对比,差异无统计学意义(t=0.083,P=0.934),治疗后,研究组积分明显低于对照组,差异有统计学意义(t=13.525,P=0.001)。研究组总有效率(94.44%)明显高于对照组(77.78%),差异有统计学意义(χ^(2)=4.181,P=0.041)。两组肩周血流动力学对比,治疗前肩胛下动脉和腋动脉流速,差异无统计学意义(t=0.036、0.012,P=0.971、0.990);治疗2周后,研究组血流速度明显快于对照组,差异有统计学意义(t=3.640、2.684,P=0.001、0.009)。结论针灸推拿治疗可以有效减轻桡骨远端骨折所致肩周炎患者的疼痛症状,改善肩周局部血液循环障碍,提高患者的治疗效果,临床应用价值显著。

Objective To investigate the clinical effect of acupuncture and massage for patients with frozen shoulder caused by fracture of the distal radius.Methods A study was conducted on patients with distal radius fractures admitted to this hospital from December 2017 to December 2019.Among them,72 patients with periarthritis of the shoulder were conveniently selected as the research objects,and 72 patients were divided into control group and study according to the double-blind method.Group,36 cases each.The control group patients were given TCM massage treatment,and the study group patients were given acupuncture treatment on this basis,and the treatment effects of the two groups were compared.Results There was no statistically significant difference in pain scores between the two groups before treatment(t=0.091,P=0.928);but after 1 and 2 weeks of treatment,the pain scores of the study group were significantly lower than those of the control group,the difference was statistically significant(t=19.511,24.271,P=0.001,0.001).Compared the scores of TCM syndromes between the two groups,there was no statistically significant difference before treatment(t=0.083,P=0.934).After treatment,the scores of the study group were significantly lower than those of the control group,the difference was statistically significant(t=13.525,P=0.001).Compared the treatment effects of the two groups,the total effective rate of the study group(94.44%)was significantly higher than that of the control group(77.78%),the difference was statistically significant(χ^(2)=4.181,P=0.041).Comparison of peri-shoulder hemodynamics in the two groups showed that there was no statistically significant difference in the flow velocity of the subscapular artery and axillary artery before treatment(t=0.036,0.012,P=0.971,0.990);after 2 weeks of treatment,the blood flow rate in the study group was significantly faster than the control group,the difference was statistically significant(t=3.640,2.684,P=0.001,0.009).Conclusion Acupuncture and massage therapy can effectively alleviate the pain symptoms of patients with frozen shoulder caused by fractures of the distal radius,improve the local blood circulation disorder around the shoulder,and improve the treatment effect of patients.The clinical application value is significant.
