探讨自拟健脾清热汤治疗溃疡性结肠炎脾虚湿热证的应用

Exploration on the Application of Self-made Jianpi Qingre Decoction in the Treatment of Ulcerative Colitis with Spleen Deficiency and Damp-heat Syndrome

目的探析自拟健脾清热汤在溃疡性结肠炎脾虚湿热证中的应用意义。方法前瞻性选取该院2019年1月—2020年1月收治的溃疡性结肠炎脾虚湿热证患者80例作为研究对象。随机将应用常规西医治疗的40例患者设立为对照组,将同时应用自拟健脾清热汤治疗的40例患者设立为观察组,疗程结束时开展指标对比,评价各自疗效、炎症因子、不良反应等。结果观察组总有效率为97.5%,高于对照组77.5%,差异有统计学意义(χ^(2)=7.314,P=0.007)。治疗前两组炎症因子差异无统计学意义(P>0.05),治疗后观察组CRP、TNF-α、IL-1β依次为(7.22±2.01)mg/L、(23.45±4.2)ng/L、(203.4±33.12)pg/mL,数值低于对照组的(9.04±1.24)mg/L、(27.88±5.01)ng/L、(231.52±42.08)pg/mL,观察组IL-4水平(7.02±1.77)ng/L,高于对照组(6.22±1.55)ng/L,差异有统计学意义(t=4.874、4.286、3.315、2.151,P<0.05)。两组均无不良反应。结论溃疡性结肠炎脾虚湿热证患者采用自拟健脾清热汤进行治疗,疗效显著,对患者炎症因子、症状改善有积极意义,安全性高。

Objective To explore the application significance of self-made Jianpi Qingre Decoction in ulcerative colitis with spleen deficiency and damp-heat syndrome.Methods Prospectively selected 80 patients with spleen deficiency and damp-heat syndrome of ulcerative colitis admitted to the hospital on January 2019 to January 2020 as study subjects.Randomly set 40 patients treated with conventional Western medicine as the control group,and set 40 patients treated with self-made Jianpi Qingre Decoction at the same time as the observation group.At the end of the treatment course,compared indicators to evaluate their respective curative effects,inflammatory factors,and symptom scores.Results The total effective rate in the observation group was 97.5%,which was higher than 77.5%in the control group.The difference was statistically significant(χ^(2)=7.314,P=0.007).Before treatment,the inflammatory factors in the two groups were equivalent,the difference was not statistically significant(P>0.05).After treatment,the CRP,TNF-α,and IL-1βof the observation group were(7.22±2.01)mg/L,(23.45±4.2)ng/L,(203.4±33.12)pg/mL,the value was lower than the control group(9.04±1.24)mg/L,(27.88±5.01)ng/L,(231.52±42.08)pg/mL,IL-4 of the observation group was(7.02±1.77)ng/L,higher than the control group(6.22±1.55)ng/L,the difference was statistically significant(t=4.874,4.286,3.315,2.151,P<0.05).There were no adverse reactions in both groups.Conclusion Patients with ulcerative colitis with spleen deficiency and damp-heat syndrome are treated with self-made Jianpi Qingre Decoction,which has significant curative effect,has positive significance for the improvement of inflammatory factors and symptoms,and has high safety.
