比较内镜下黏膜切除术和内镜下黏膜下剥离术治疗结肠粗蒂息肉对患者预后的影响

Comparison of the Effects of Endoscopic Mucosal Resection and Endoscopic Submucosal Dissection on the Prognosis of Patients with Coarse Pedicle Polyps in the Colon

目的对比内镜下黏膜切除术和内镜下黏膜下剥离术治疗结肠粗蒂息肉对患者预后的影响。方法研究选取该院的100例结肠粗蒂息肉患者进行探讨,患者在2018年5月—2020年6月期间入院,按照入院的时间顺序对患者分组,对照组采用内镜下黏膜下剥离术治疗,观察组采用内镜下黏膜切除术治疗,对比两组的临床疗效以及预后疗效。结果观察组患者治疗有效率为98%,对照组治疗有效率为96%,组间差异无统计学意义(χ^(2)=0.000,P>0.05);观察组手术时间是(19.83±6.38)min,止血夹数量为(5.12±0.27)个,优于对照组,差异有统计学意义(t=13.063、13.717,P<0.05)。结论内镜下黏膜切除术和内镜下黏膜下剥离术治疗结肠粗蒂息肉均可以获得比较理想的效果,术后并发症控制效果较好,但前者操作比较简单,止血操作较少,说明患者出血风险低,建议推广。

Objective To compare the effects of endoscopic mucosal resection and endoscopic submucosal dissection on the prognosis of patients with thick pedicle polyps.Methods The study selected 100 patients with thick pedicle polyps of the colon in the hospital for discussion.The patients were admitted to the hospital from May 2018 to June 2020.The patients were divided into groups according to the time sequence of admission.The control group was treated with endoscopic submucosal dissection for surgical treatment,the observation group was treated with endoscopic mucosal resection,and the clinical efficacy and prognostic efficacy of the two groups were compared.Results The effective rate of treatment was 98%in the observation group,and the effective rate of treatment was 96%in the control group.The difference between the two groups was not statistically significant(χ^(2)=0.000,P>0.05);the operation time of the observation group was(19.83±6.38)min,the number of hemostatic clips was(5.12±0.27),which was better than the control group,the difference was statistically significant(t=13.063,13.717,P<0.05).Conclusion Both endoscopic mucosal resection and endoscopic submucosal dissection for the treatment of colonic polyps with thick pedicles can achieve satisfactory results,and the postoperative complications are better controlled,but the former is relatively simple to operate and less hemostatic,which indicates that the risk of bleeding in patients is low,and it is recommended to promote it.
