摘要 目的探讨高迁移率族蛋白1(HMGB1)在直肠癌中的表达情况及与患者预后的关系。方法收集78例直肠癌患者的直肠癌组织及癌旁组织,检测HMGB1的表达情况,分析直肠癌组织中HMGB1与免疫淋巴细胞浸润情况的关系及HMGB1与患者临床特征的关系;对患者术后随访40个月,统计生存时间。结果直肠癌组织中HMGB1表达情况高于癌旁组织,差异有统计学意义(P﹤0.05)。不同病理分期、肿瘤结节情况、CD3^(+)浸润情况、CD45RO^(+)浸润情况、脉管癌栓或神经侵犯情况、淋巴结转移情况、远处转移情况的直肠癌患者的直肠癌组织中HMGB1蛋白表达情况比较,差异均有统计学意义(P﹤0.05)。HMGB1核表达的直肠癌组织中淋巴细胞CD3^(+)、CD45RO^(+)高密度浸润比例较高,差异均有统计学意义(P﹤0.05)。HMGB1核表达、HMGB1高表达、CD3^(+)低密度浸润、CD45RO^(+)低密度浸润患者的生存时间分别短于HMGB1核质共表达、HMGB1低/无表达、CD3^(+)高密度浸润、CD45RO^(+)高密度浸润患者,差异均有统计学意义(P﹤0.05)。多因素分析发现,HMGB1表达部位、病理分期、CD3^(+)浸润情况、CD45RO^(+)浸润情况均为直肠癌患者生存时间的影响因素(P﹤0.05)。结论临床对于直肠癌患者HMGB1表达及免疫淋巴细胞动态监测,能够帮助了解患者的预后情况,为调整治疗方案提供一定的帮助。 Objective To investigate the expression of high mobility group box 1(HMGB1)in rectal cancer and its relationship with the prognosis of patients.Method A total of 78 cases of rectal cancer were selected,and their cancer tissues and adjacent tissues were collected.The expression of HMGB1 was detected,and the relationship between HMGB1 and immune lymphocyte infiltration in cancer tissues and the relationship between HMGB1 and the patients’clinical characteristics were also analyzed.The patients were followed up for 40 months after surgery,and the overall survival was counted.Result The expression of HMGB1 in rectal cancer tissues was higher than that in normal tissues,and the difference was statistically significant(P<0.05).The HMGB1 protein expression in rectal cancer tissues of patients with different pathological stages,tumor nodules,CD3^(+) infiltration,CD45RO^(+) infiltration,vascular tumor thrombus or nerve invasion,lymph node metastasis,and distant metastasis were statistically significant different(P<0.05).The ratio of lymphocytes CD3^(+) and CD45RO^(+) high-density infiltration in rectal cancer tissues with HMGB1 nuclear expression were higher,and the differences were statistically significant(P<0.05).The overall survival of patients with HMGB1 nuclear expression,HMGB1 high expression,CD3^(+) low-density infiltration,and CD45RO^(+) low-density infiltration were worse than those of patients with HMGB1 nuclear and cytoplasmic co-expression,HMGB1 low/no expression,CD3^(+) high-density infiltration,and CD45RO^(+) high-density infiltration,respectively,the differences were statistically significant(P<0.05).Multivariate analysis found that the location of HMGB1 expression,pathological stage,CD3^(+) invasion,and CD45RO^(+) invasion were all independent risk factors for the survival of patients with rectal cancer(P<0.05).Conclusion Dynamic monitoring of HMGB1 expression and immune lymphocyte of patients with rectal cancer in clinical practice could do help predict the prognosis of patients and further provide some advice for the adjustment of the treatment regimen.
