基于公立医院视角的国家组织药品集中采购政策效果分析——以上海市某三甲医院为例

Analysis of the effects of national centralized drug procurement policy from the perspective of public hospitals—A case study of class a tertiary hospital in shanghai

国家组织药品集中采购政策是深化医改的重要体现,上海市在开展试点工作的同时制定了相应的配套政策。本研究以上海市某三甲医院对国家组织药品集中采购政策执行情况为例,从合理用药、收支结构、费用控制等多维度对药品集中采购在公立医院产生的政策效果进行系统分析。结果显示2019年4月—9月对比去年同期,中选药品处方量上升143.77%,原研药处方量下降77.0%,以阿托伐他汀为例,中选药物药品用量、用药频度、以及药品费用对比去年同期分别上升865.72%、911.42%和23.75%。医院药品总费用对比去年同期下降53.29%,药占比下降1.09%,这些指标,说明实施集中采购政策后,药品降价卓有成效。本研究对药品集中采购政策改革效果进行了综合评价,为进一步完善药品集中采购办法提供参考。

The national centralized drug procurement policy is an important manifestation of the deepening of medical reform.Shanghai has formulated corresponding supporting policies while carrying out pilot work.This study takes a third-class hospital in Shanghai as an example and systematically analyzes the effects of national centralized drug procurement policy in public hospitals from multiple perspectives such as rational drug use,revenue and expenditure structure,and cost control.When compare April to September in 2019 to the same period last year,the results show that the number of prescriptions for selected drugs increased by 143.77%,while the number of prescriptions for original research drugs decreased by 77.0%.Taking atorvastatin as an example,the amount of selected drugs,defined daily dose system,and drug costs increased 865.72%,911.42%,and 23.75%,respectively.The total cost of medicines in hospitals dropped by 53.29%,and the proportion of medicines dropped by 1.09%,from the same period last year.These results indicate that the price of medicines has been reduced.This study conducted a comprehensive evaluation of the effect of national centralized drug procurement policy,and provided a reference for further improvement of the policy.
