AIDET沟通模式在家庭签约社区老年糖尿病患者中的应用

Application of AIDET communication model in elderly diabetic patients in family contract community

目的研究AIDET沟通模式对家庭签约社区老年糖尿病患者进行健康管理的应用效果。方法选择深圳市福永人民医院下辖的凤凰山社康、和平社康建立健康档案的老年糖尿病患者共120例,随机分为对照组(n=60)与观察组(n=60),对照组采用传统沟通模式进行健康管理,观察组则采用AIDET沟通模式进行健康管理,对比两组干预后的肾小球滤过率(eGFR)、血糖指标、服药依从性及满意度。结果干预6个月及1年后,两组eGFR、空腹血糖(FPG)、餐后2 h血糖(2 hPG)及糖化血红蛋白(HbA1c)均较干预前改善(P<0.05),观察组eGFR、血糖达标率显著高于对照组(P<0.05),观察组FPG、2hPG、HbA1c显著低于对照组(P<0.05)。服药依从性方面,对照组为75%,观察组为96.67%,观察组服药依从性显著高于对照组(P<0.05)。满意度方面,对照组为78.33%,观察组为98.33%,观察组满意度显著高于对照组(P<0.05)。结论采用AIDET沟通模式对家庭签约社区老年糖尿病患者进行健康管理能有效延缓或减少并发症的发生,是一种有效的慢性病管理模式,值得在广大社区卫生服务中心推广应用。

Objective To study the application effect of the AIDET communication model on the health management of elderly diabetic patients in family contract community.Methods A total of 120 elderly diabetic patients who established health records from Phoenix Mountain community health center and Heping community health center under Shenzhen Fuyong People’s Hospital were randomly divided into two groups:a control group(n=60)and an observation group(n=60).The control group used Traditional communication mode for health management,while the observation group used AIDET communication mode for health management.The glomerular filtration rate(e GFR),blood glucose index,medication compliance and satisfaction of both groups after intervention were compared.Results After a 6-month and 1-year interventions,the eGFR,fasting plasma glucose(FPG),2h postprandial blood glucose(2hPG)and glycosylated hemoglobin(HbA1c)in both groups were improved compared with those before intervention(P<0.05).The eGFR and blood glucose compliance rate of observation group were significantly higher than the control group(P<0.05),the FPG,2hPG,HbA1c of observation group were significantly lower than the control group(P<0.05).In terms of medication compliance,the control group was 75%while the observation group was 96.67%,and the medication compliance of the observation group was significantly higher than the control group(P<0.05).In terms of satisfaction,the control group was 78.33%and the observation group was 98.33%,and the satisfaction of observation group was significantly higher than control group(P<0.05).Conclusion Adopting the AIDET communication model to manage the health of elderly diabetic patients in family contract community can effectively delay or reduce the occurrence of complications.It is an effective chronic disease management model and worthy of being promoted and applied in most community health service centers.
