自我效能感在宫颈上皮内瘤变患者疾病不确定感与焦虑间的中介作用

Mediating effect of self-efficacy on disease uncertainty and anxiety in patients with cervical intraepithelial neoplasisa

目的探讨宫颈上皮内瘤变患者疾病不确定感、自我效能感与焦虑之间的关系。方法应用广泛性焦虑量表、一般自我效能感量表和疾病不确定感量表对青岛市某三级甲等医院妇科的288例宫颈上皮内瘤变患者进行问卷调查。结果宫颈上皮内瘤变患者的焦虑总分为(6.83±2.17)分,自我效能感总分为(24.28±5.48)分,疾病不确定感总分为(61.85±12.83)分;宫颈上皮内瘤变患者的焦虑得分与自我效能感呈负相关,与疾病不确定感呈正相关,其自我效能感与疾病不确定感呈负相关;宫颈上皮内瘤变患者的疾病不确定感对焦虑具有正向预测作用(r=0.794,P<0.01),自我效能感是宫颈上皮内瘤变患者疾病不确定感与焦虑之间的中介变量,中介效应占总效应的38.90%。结论宫颈上皮内瘤变患者的疾病不确定感水平越低,自我效能感水平越高,其焦虑水平越低,自我效能感在宫颈上皮内瘤变患者疾病不确定感对焦虑的影响过程中发挥部分中介效应,降低疾病不确定感、提升自我效能感是降低宫颈上皮内瘤变患者焦虑水平的有效途径。

Objective To investigate the relationship among disease uncertainty,self-efficacy and anxiety in patients with cervical intraepithelial neoplasisa.Methods Using Generalized Anxiety Disorder,General Self-efficacy Scale and Mishel's Uncertainty in Illness Scale to investigate 288 patients with cervical intraepithelial neoplasia in the Department of Gynecology of a 3A-level hospital in Qingdao.Results The total score of anxiety,self-efficacy and disease uncertainty was(6.83±2.17),(24.28±5.48)and(61.85±12.83),respectively.The patients'anxiety was negatively correlated with self-efficacy and positively correlated with disease uncertainty.There was a negative correlation between self-efficacy and disease uncertainty.The sense of disease uncertainty had a positive predictive effect on their anxiety.Anxiety score has a positive predictive effect(r=0.791,P<0.01).The self-efficacy was an intermediate variable between the disease uncertainty and anxiety,whose effect accounted 38.90%.Conclusion The lower the level of disease uncertainty,the higher degree of self-efficacy and the lower degree of anxiety in patients with cervical intraepithelial neoplasias.Self-efficacy plays an intermediary role in the effect of disease uncertainty on their anxiety.To reduce patients'disease uncertainty and improve self-efficacy is an effective way to reduce their anxiety.
