Bakri球囊填塞术联合米非司酮治疗凶险性前置胎盘的效果研究

Study on effect of Bakri balloon tamponation combined with mifepristone in the treatment of dangerous placenta previa

目的分析Bakri球囊填塞术联合米非司酮治疗凶险性前置胎盘的临床效果。方法40例凶险性前置胎盘患者,按照入院时间不同分为对照组和研究组,每组20例。对照组实施Bakri球囊填塞术治疗,研究组实施Bakri球囊填塞术联合米非司酮治疗。对比两组患者术中出血量、术后2 h阴道出血量、术后24 h阴道出血量、治疗时间、术后住院时间、血红蛋白水平、血小板水平、中心静脉压。结果研究组术中出血量及术后2、24 h阴道出血量分别为(515.9±43.5)、(108.8±13.8)、(45.6±7.4)ml,均少于对照组的(575.2±49.7)、(175.6±26.9)、(91.7±11.9)ml,差异有统计学意义(P<0.05)。研究组治疗时间、术后住院时间均短于对照组,血红蛋白、血小板、中心静脉压水平均高于对照组,差异有统计学意义(P<0.05)。结论对凶险性前置胎盘患者采用Bakri球囊填塞术联合米非司酮治疗可以减少术中及术后出血量,缩短治疗及住院时间,改善血常规指标及中心静脉压。

Objective To analyze the clinical effect of Bakri balloon tamponation combined with mifepristone in the treatment of dangerous placenta previa.Methods A total of 40 patients with dangerous placenta previa were divided into control group and research group according to different admission time,with 20 cases in each group.The control group was treated with Bakri balloon tamponation,and the research group was treated with Bakri balloon tamponation combined with mifepristone.The intraoperative blood loss,vaginal blood loss at 2 h after surgery,vaginal blood loss at 24 h after surgery,treatment time,postoperative hospitalization time,hemoglobin level,platelet level,central venous pressure were compared between the two groups.Results The intraoperative blood loss,vaginal blood loss at 2 and 24 h after surgery of the research group were(515.9±43.5),(108.8±13.8)and(45.6±7.4)ml,which were less than(575.2±49.7),(175.6±26.9)and(91.7±11.9)ml of the control group,and the difference was statistically significant(P<0.05).The treatment time and postoperative hospitalization time of the research group were shorter than those of the control group,and hemoglobin,platelet and central venous pressure were higher than those of the control group.All the difference was statistically significant(P<0.05).Conclusion Combination of Bakri balloon tamponation and mifepristone for patients with dangerous placenta previa can reduce the intraoperative and postoperative blood loss,shorten treatment and hospitalization time,and improve blood routine indicators and central venous pressure.
