强化功能训练对肩袖损伤关节镜手术患者上肢功能和康复效果的影响分析

Analysis of the effect of intensive functional training on upper limb function and rehabilitation of patients with rotator cuff injury undergoing arthroscopic surgery

目的分析强化功能训练对肩袖损伤关节镜手术患者上肢功能和康复效果的影响。方法选取2018年10月至2019年10月合肥市第一人民医院收治的80例肩袖损伤患者为研究对象,采用随机数字表法将入选患者分别纳入强化训练组和常规训练组,每组各40例,均行关节镜手术治疗,术后分别予以强化功能训练和常规康复训练。比较两组患者术前和术后12个月肩关节评分、肩关节活动范围和生活质量。结果两组患者切口均一期愈合,均未见关节不稳、肩关节僵硬等并发症发生,且均获得有效随访。两组患者术后12个月Constant-Murley量表各项目评分和总分、健康调查量表36(36-item short form health survey,SF-36)各项目评分均显著高于本组术前(均P<0.05),肩关节各方向活动度均显著大于本组术前(均P<0.05);强化训练组患者术后12个月Constant-Murley量表各项目评分和总分、SF-36各项目评分均显著高于常规训练组(均P<0.05),肩关节各方向活动度均显著大于常规训练组(均P<0.05)。结论强化功能训练能够改善肩袖损伤患者关节镜术后肩关节功能、提高肩关节活动度并改善患者生活质量,对于促进患者上肢功能恢复、改善康复效果有着积极作用。

Objective To analyze the effect of intensive functional training on upper limb function and rehabilitation of patients with rotator cuff injury undergoing arthroscopic surgery.Method From October 2018 to October 2019,80 patients with rotator cuff injury in the First People’s Hospital of Hefei City were selected as the subjects.They were randomly divided into intensive training group and conventional training group,with 40 cases in each group.All patients were treated with arthroscopic surgery,and were given intensive functional training and routine rehabilitation training after operation respectively.The changes of shoulder joint score,range of motion of shoulder joint and quality of life were compared between the two groups before and 12 months after operation.Result The incisions of the two groups were healed at the first stage,and there were no complications such as joint instability and shoulder stiffness,all patients were followed up effectively.The scores and total scores of each item in Constant-Murley scale and each item in 36-item short form health survey in the two groups at 12 months after operation were significantly higher than those before operation in this group(all P<0.05),the activities of shoulder joints in all directions were significantly larger than those before operation in this group(all P<0.05).The scores and total scores of each item in Constant-Murley scale and each item in SF-36 in intensive training group 12 months after operation were significantly higher than those in conventional training group(all P<0.05),and the activities in shoulder joints in all directions were significantly larger than those in conventional training group(all P<0.05).Conclusion Intensive functional training can improve the shoulder joint function of patients with rotator cuff injury after arthroscopic surgery,improve the range of motion of shoulder joint and improve the quality of life of patients,which has a positive role in promoting the recovery of upper limb function and improving the rehabilitation effect.
