海曼联合子宫下段环形捆扎术在产后出血的应用

Application of Hyman combined with lower uterine ring banding in postpartum hemorrhage

目的研究改良海曼式缝合联合子宫下段环形捆扎术在产后出血中的应用效果。方法随机选取2018年10月—2020年12月间在兴国县人民医院分娩后发生产后出血患者60例为研究对象,按照分娩日期的前后分为2组,即对照组(n=30)与海曼组(n=30)。对照组患者予以传统宫腔填塞术止血,海曼组患者予以改良海曼式缝合联合子宫下段环形捆扎术止血。统计2组患者的出血量、操作时间、彻底止血时间与住院时间、月经恢复时间。结果与对照组患者相比,海曼组患者出血量、操作时间、彻底止血时间均明显更低(P<0.05)。与对照组患者相比,海曼组患者住院时间、月经恢复时间均明显更短(P<0.05)。结论改良海曼式缝合联合子宫下段环形捆扎术治疗产后出血,可取得较好的止血效果,患者的彻底止血时间更短,生理功能可更迅速地恢复,临床应用价值显著。

Objective To study the effect of modified Hyman suture combined with lower uterine ring banding in the treatment of postpartum hemorrhage.Methods 60 patients with postpartum hemorrhage in our hospital from October 2018 to December 2020 were randomly selected as the research objects,and they were divided into two groups according to the delivery date,namely control group(n=30)and Hyman group(n=30).Patients in the control group were treated with traditional uterine packing to stop bleeding,while patients in the Hyman group were treated with modified Hyman suture combined with circular banding of lower uterine segment to stop bleeding.The bleeding volume,operation time,complete hemostasis time,hospitalization time and menstrual recovery time of the two groups were statistically analyzed.Results compared with the control group,the blood loss,operation time and complete hemostasis time of the Heiman group were significantly lower(P<0.05).Compared with the control group,the data results of hospitalization time and menstrual recovery time in the Hyman group were significantly shorter(P<0.05).Conclusion In the treatment of postpartum hemorrhage,the combination of modified Hyman suture and circular banding of lower uterine segment can achieve a higher hemostatic effect.Patients have less blood loss,shorter time for complete hemostasis,and shorter hospital stay.At the same time,the physiological function of patients has been more rapid recovery,and the clinical application value is significant.
