青光眼患者个案管理系统的开发和应用效果研究

Study on the development and application effect of case management system for glaucoma patients

目的设计并研发青光眼个案管理系统并研究其应用效果。方法2018年1月~2019年12月青光眼个案管理系统研发前后随机纳入青光眼患者各1000例,对比分析个案管理系统研发前后患者的检查完成度、设定的靶眼压达标率、随访依从性、随访率、患者满意度共5项敏感指标的结果。结果青光眼个案管理系统研发后,诊疗前指定项目的检查率从70%提升到97%;患者靶眼压的达标率从0%上升到了67%;随访依从性从28%提升到67.5%;随访率从30%提升到73.4%;患者满意度从95%提高到99.5%,差异均有统计学意义(P<0.001)。结论本系统的研发和使用为青光眼慢性眼病延续护理及终身随访提供了有力的系统保障;5项敏感指标的持续监测,有效遏制了青光眼病情的进一步发展,为精准诊疗和护理做出了积极的贡献。

Objective To design and develop a case management system for glaucoma patients and study its application effects.Methods From January 2018 to December 2019,1000 glaucoma patients were randomly included before and after the development of glaucoma case management system.The results of five sensitive indicators before and after the development of the case management system were compared and analyzed,including the completion degree of the examination,the rate reaching the target intraocular pressure,the compliance with follow-up,the follow-up rate and the patients′satisfaction.Results After the development of the glaucoma case management system,the inspection rate of designated items before diagnosis and treatment increased from 70%to 97%.The rate reaching target intraocular pressure increased from 0%to 67%.The compliance with follow-up increased from 28%to 67.5%.The follow-up rate increased from 30%to 73.4%;and patients′satisfaction increased from 95%to 99.5%,and the differences were statistically significant(P<0.001).Conclusion The development and application of the system provide a powerful systematic guarantee for the continuous nursing and lifelong follow-up of glaucoma chronic ophthalmopathy.The continuous monitoring of the five sensitive indexes has effectively curbed the further development of glaucoma and made positive contributions to accurate diagnosis,treatment and nursing.
