2型糖尿病围手术期胰岛素泵治疗疗效及安全性分析

Analysis of the efficacy and safety of insulin pump in the perioperative period of type 2 diabetes

目的:观察2型糖尿病围手术期胰岛素泵治疗的疗效及安全性。方法:选取2008年1月-2020年6月行手术治疗的2型糖尿病患者68例,随机分为两组,各34例。A组围术期接受皮下注射胰岛素治疗;B组围术期接受胰岛素泵治疗。比较两组临床疗效及安全性。结果:B组治疗3 d后空腹血糖、餐后2 h血糖水平均低于A组,差异有统计学意义(P<0.05);B组术前血糖控制时间、胰岛素用量、术中血糖及术后血糖水平均显著低于A组,差异有统计学意义(P<0.05);B组不良反应发生率显著低于A组,差异有统计学意义(P<0.05)。结论:在2型糖尿病患者围手术期应用胰岛素泵治疗,可有效控制患者血糖水平,避免血糖出现明显波动,提高手术治疗的安全性,且可减少治疗期间的不良反应发生率。

Objective:To observe the efficacy and safety of insulin pump therapy for type 2 diabetes during perioperative period.Methods:From January 2008 to June 2020,68 patients with type 2 diabetes who underwent surgical treatment were selected,they were randomly divided into the two groups with 34 cases in each group.The group A received subcutaneous insulin therapy during the perioperative period.The group B received insulin pump therapy during the perioperative period.The clinical efficacy and safety of the two groups were compared.Results:The fasting blood glucose level and the blood glucose level 2 h postprandial in the group B were lower than the group A after 3 days of treatment,the difference was statistically significant(P<0.05).The preoperative blood glucose control time,insulin dosage,intraoperative blood glucose and postoperative blood glucose level in the group B were significantly lower than those in the group A,the difference was statistically significant(P<0.05).The incidence of adverse reactions in group B was significantly lower than that of the group A,the difference was statistically significant(P<0.05).Conclusion:The application of insulin pump therapy in patients with type 2 diabetes during perioperative period can effectively control the patient’s blood glucose level,avoid obvious fluctuations in blood glucose,improve the safety of surgical treatment,and reduce the incidence of adverse reactions during treatment.
