中性粒细胞/淋巴细胞比值与原发性高血压患者颈动脉内膜中层厚度的相关性研究

Study on the correlation between neutrophil/lymphocyte ratio and carotid artery intima-media thickness in patients with essential hypertension

目的:探讨中性粒细胞/淋巴细胞比值(NLR)与原发性高血压患者颈动脉内膜中层厚度(CIMT)的关系。方法:2018年5月-2019年12月收治新发未经治疗的原发性高血压患者300例,均检测生化指标及血常规并计算NLR;超声测量CIMT,并根据CIMT值分为CIMT增厚组和CIMT正常组,比较两组相关指标差异,并分析NLR与CIMT的相关性。结果:CIMT增厚组年龄、收缩压(SBP)、舒张压(DBP)、体重指数(BMI)、空腹血糖(FBG)、血尿酸(UA)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、超敏C-反应蛋白(hs-CRP)、CIMT及NLR水平均明显高于CIMT正常组,差异有统计学意义(P<0.05)。年龄、SBP、DBP、BMI、FBG、UA、TG、总胆固醇(TC)、LDL-C、hs-CRP及NLR水平均与CIMT增厚组CIMT水平呈正性相关(P<0.05)。Logistic回归分析,NLR是原发性高血压患者CIMT增厚的独立危险因素(P<0.05)。结论:原发性高血压患者NLR水平变化可能反映了CIMT动脉粥样硬化的变化;NLR可能是原发性高血压患者CIMT增厚的独立危险因素。

Objective:To investigate the relationship between neutrophil/lymphocyte ratio(NLR)and carotid intima-media thickness(CIMT)in patients with essential hypertension.Methods:From May 2018 to December 2019,300 newly untreated patients with essential hypertension were admitted.Biochemical indicators and blood routine were tested and NLR was calculated.CIMT was measured by ultrasound.The patients were divided into the CIMT thickening group and the CIMT normal group according to the CIMT value.The differences of related indexes between the two groups were compared and the correlation between NLR and CIMT was analyzed.Results:The age,systolic blood pressure(SBP),diastolic blood pressure(DBP),BMI,fasting blood glucose(FBG),serum uric acid(UA),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),high sensitivity C-reactive protein(hs-CRP),CIMT and NLR levels of the CIMT thickening group were significantly higher than those of the CIMT normal group,the difference was statistically significant(P<0.05).Age,SBP,DBP,BMI,FBG,UA,TG,total cholesterol(TC),LDL-C,hs-CRP and NLR levels were positively correlated with CIMT level in the CIMT thickening group(P<0.05).Logistic regression analysis showed that NLR was an independent risk factor for CIMT thickening in patients with essential hypertension(P<0.05).Conclusion:The changes of NLR level in patients with essential hypertension may reflect the changes of atherosclerosis in CIMT,and NLR may be an independent risk factor for CIMT thickening in patients with essential hypertension.
