四藤一仙汤治疗腰椎术后疼痛综合征的疗效分析

Analysis of Efficacy of Siteng Yixian Decoction(四藤一仙汤)in Treatment of Failed Back Surgery Syndrome

目的探究腰椎术后出现疼痛综合征的患者应用四藤一仙汤治疗后的临床效果。方法选取2018年1月——2019年12月因腰椎相关疾病住院行手术治疗的患者1546例,其中术后出现腰痛患者168例,采用随机数字法将患者分为3组,对照组1采用塞来昔布胶囊治疗、对照组2采用针刀治疗、治疗组采用四藤一仙汤内服治疗,比较3组VAS评分,腰椎JOA评分,各组有效率。结果治疗3 d后,3组患者VAS评分下降,腰椎JOA评分上升,但治疗组VAS评分与JOA评分较两对照组改变较小,3组差异具有统计学意义,P<0.05,总有效率低于两对照组;治疗7 d后,对照组2 VAS评分低于对照组1,治疗组VAS评分与对照组2相接近,腰椎JOA评分高于两对照组,差异具有统计学意义,P<0.05,治疗组总有效率呈上升趋势;治疗12 d后,治疗组VAS评分显著低于两对照组,JOA评分明显上升,差异具有统计学意义,P<0.05,总有效率达87.5%(49/56),显著优于两对照组。结论四藤一仙汤治疗腰椎术后疼痛综合征较传统治疗方法效果显著,同时改善患者功能障碍,提高生活质量,在临床有广泛的应用前景。

Objective To investigate the clinical effect ofSiteng Yixian Decoction(四藤一仙汤) in patients with pain syndrome after lumbar spine surgery.Methods A total of 1546 patients who were hospitalized for lumbar spine related diseases from January 2018 to December 2019 were selected.Among them, 168 patients developed low back pain after operation.The patients were divided into 3 groups by random number method.The control group 1 was treated with Celecoxib Capsules.The control group 2 was treated withneedle-knife.The treatment group was treated with Siteng YixianDecoction, and the VAS score and the JOA score of the lumbar spine were compared among the three groups.Results After 3 days of treatment, the VAS scores of the three groupsdecreased and the JOA scores of the lumbar spine increased, but the VAS score and the JOA score of the treatment group were less changed than those of the two control groups.The difference among the three groups was statistically significant(P<0.05),and the total effective rate was lower those of the two control groups.After 7 days of treatment, the VAS score of the control group 2 was lower than that of the control group 1 andthe VAS score of the treatment group was close to that of the control group 2,and the JOA score of the lumbar spine was higher than those of the two control groups.The total effective rate in the treatment group showed an upward trend.After 12 days of treatment, the VAS score in the treatment group was significantly lower than those of the two control groups, and the JOA score was significantly increased.The difference was statistically significant.Conclusion Siteng Yixian Decoction has a significantly better effect than traditional treatment methods on treating lumbar spinal pain syndrome, and it can improve the quality of life and dysfunction of patients.It has broad application prospects in clinic.
