温阳益肾汤联合磷酸西格列汀治疗糖尿病肾病的临床效果

Clinical Effect of Wenyang Yishen Decoction(温阳益肾汤)Combined with Sitagliptin Phosphate on Diabetic Nephropathy

目的观察温阳益肾汤联合磷酸西格列汀治疗糖尿病肾病的临床效果。方法将70例糖尿病肾病患者随机分成观察组35例及对照组35例,对照组采用常规方法治疗,观察组在对照组的基础上加入温阳益肾汤和磷酸西格列汀治疗,观察两组患者治疗前后的空腹及餐后2 h血糖、血肌酐、尿素氮等指标,中医症状积分及治疗有效率。结果经过治疗后,观察组的空腹血糖为(6.17±0.71)mmol/L、低于对照组(6.96±0.83)mmol/L;餐后血糖(餐后2 h)(8.30±1.15)mmol/L、低于对照组(9.97±1.70)mmol/L;糖化血红蛋白(6.68±0.34)mmol/L、低于对照组(8.02±0.64)mmol/L;血清肌酐(163.32±42.64)μmol/L,低于对照组(229.88±56.52)μmol/L;尿素氮(6.13±0.64)mmol/L、低于对照组(7.79±0.68)mmol/L;24 h尿蛋白定量为(2.47±1.23)g/24 h,低于对照组(3.23±1.37)g/24 h;中医证候积分为(8.59±2.51)分,低于对照组(12.26±3.00)分;观察组显效13例、有效19例、无效3例,有效率为91.43%(32/35);对照组显效7例、有效16例、无效12例,有效率为65.71%(23/35),差异均具有统计学意义(P<0.05)。结论温阳益肾汤联合磷酸西格列汀在控制糖尿病肾病患者的血糖、改善肾功能,缓解临床症状方面,均取得不错的疗效,值得推广应用。

Objective To evaluate the clinical effect of Wenyang Yishen Decoction(温阳益肾汤) combined with sitagliptin phosphate on diabetic nephropathy.Methods Seventy diabetic nephropathy patients were randomly divided into observation group(35 cases) and control group(35 cases).The control group was treated with conventional methods.The observation group added Wenyang Yishen Decoction and sitagliptin phosphate on the basis of the control group, and we observed the patients′ fasting, 2 h postprandial blood glucose, blood creatinine, urea nitrogen and other indicators, TCM symptom score and treatment efficiency before and after treatment.Results After treatment, the fasting blood glucose in the observation group was(6.17±0.71)mmol/L,lower than that of the control group [(6.96±0.83)mmol/L].The postprandial blood glucose(2 hours after meals) was(8.30±1.15)mmol/L,lower than that of the control group[(9.97±1.70)mmol/L].The glycated hemoglobin was(6.68±0.34)mmol/L,lower than that of the control group [(8.02±0.64)mmol/L].The serum creatinine was(163.32±42.64)μmol/L,lower than the that of control group[(229.88±56.52)μmol/L].The urea nitrogen was(6.13 ±0.64)mmol/L,lower than that of the control group[(7.79±0.68)mmol/L].The 24-hour urinary protein quantification was(2.47±1.23)g/24 h, lower than that of the control group(3.23±1.37)g/24 h.TCM symptom score was(8.59±2.51)points, lower than that of the control group[(12.26±3.00)points].The observation group showed 13 cases were markedly effective, 19 cases effective and 3 cases ineffective.The effective rate was 91.43%(32/35).The control group had 7 cases markedly effective, 16 cases and 12 cases invalid.The effective rate was 65.71%(23/35).The differences were statistically significant(P<0.05).Conclusion Wenyang Yishen Decoction combined with sitagliptin phosphate has achieved good results in controlling blood sugar, improving renal function and relieving clinical symptoms in patients with diabetic nephropathy, and it is worthy of popularization and application.
