中药擦洗术治疗湿热内蕴型丹毒临床观察

Clinical Observation on Treatment of Damp-heat Erysipelas with Traditional Chinese Medicine Scrubbing

目的观察中药擦洗术治疗湿热内蕴型丹毒的临床疗效。方法将74例湿热内蕴型丹毒患者随机分为治疗组和对照组各37例,在常规治疗的基础上,治疗组采用中药擦洗,对照组采用50%硫酸镁湿敷,两组均每次治疗30 min,1次/d,连续治疗2周。分别在治疗前、治疗1周后及治疗2周后观察两组患者的体温、肤色、肤温、水肿、炎症指标(白细胞计数、中性粒细胞百分比、CRP计数)情况,并通过统计学处理分析后判定临床疗效。结果治疗1月后,中药擦洗组治愈率为86.49%(32/37),硫酸镁湿敷组治愈率为75.68%(28/37),两组临床疗效比较无差异(P>0.05)。治疗1周后中药擦洗组在肤色、肤温、水肿积分方面均低于同时间点的硫酸镁湿敷组(P<0.05);治疗2周后中药擦洗组在肤色、水肿积分方面仍低于硫酸镁湿敷组(P<0.05)。两组炎症指标治疗1周后比较均具有统计学差异(P<0.05)。结论中药擦洗术治疗湿热内蕴型丹毒疗效明显,尤其在改善下肢皮肤红肿方面更为显著。

Objective To observe the clinical effect of traditional Chinese medicine scrubbing on erysipelas with damp-heat.Methods Seventy-four cases of erysipelas with damp-heat were randomly divided into treatment group and control group,37 cases in each group.On the basis of routine treatment,the treatment group was scrubbed with traditional Chinese medicine,and the control group was treated with 50%magnesium sulfate hydropathic compress.Both groups were treated for 30 minutes once a day for 2 weeks.Before treatment,1 and 2 weeks after treatment,the body temperature,skin color,skin temperature,edema and inflammation indexes(leukocyte count,neutrophil percentage,C-reactive protein count)of the two groups were observed,and the clinical efficacy was determined after statistical analysis.Results After one month of treatment,the cure rate of traditional Chinese medicine scrubbing group was 86.49%(32/37),and that of magnesium sulfate hydropathic compress group was 75.68%(28/37).There was no difference in clinical effect between the two groups(P>0.05).After one week of treatment,the skin color,skin temperature and edema scores of the traditional Chinese medicine scrubbing group were lower than those of the magnesium sulfate external application group at the same time point(P<0.05).After two weeks of treatment,the skin color and edema score of the traditional Chinese medicine scrubbing group were still lower than those of the magnesium sulfate external application group(P<0.05).There was statistical difference between the two groups after one week of treatment(P<0.05).Conclusion The curative effect of traditional Chinese medicine scrubbing on erysipelas of damp-heat type is obvious,especially in relieving the skin redness and swelling of lower limbs.
