中药冷喷结合疏经通络推拿法对早期产后乳胀及母乳喂养的影响

Study on Effect of Cold Spray of Traditional Chinese Medicine Combined with Dredging Meridians and Collaterals Tuina on Early Postpartum breast distension and Breastfeeding

目的研究中药冷喷结合疏经通络推拿法对早期产后乳胀及母乳喂养的影响,为产妇的恢复提供临床指导。方法选取2019年1月—2019年9月期间在余姚市人民医院产科分娩的90例乳房胀痛产妇作为研究对象。按照随机数表法,将其分为研究组和对照组,每组各45例。对照组采用疏经通络推拿法进行治疗,研究组在对照组的基础上结合中药冷喷法联合治疗,检测并记录两组产妇产后3 d乳房胀痛程度、乳房硬度程度、乳房充盈程度、开始泌乳时间和泌乳量情况以及产后3 d母乳喂养方式。结果产后3 d研究组产妇的乳房胀痛程度、乳房硬度程度、乳房充盈程度明显优于对照组产妇,差异具有统计学意义(P<0.05)。研究组产妇开始泌乳时间为(16.04±4.89)h,明显短于对照组产妇的(23.07±5.77)h,差异显著(P<0.05)。治疗后两组产妇的泌乳量均有明显提升(P<0.05),且治疗后24 h、48 h、72 h研究组产妇的泌乳量均明显高于对照组产妇,差异具有统计学意义(P<0.05)。经过治疗,研究组产妇母乳喂养率为80.0%(36/45),对照组产妇母乳喂养率为60.0%(27/45),研究组明显高于对照组,差异具有统计学意义(P<0.05)。结论中药冷喷结合疏经通络推拿法早期干预有利于改善产妇产后乳胀疼痛感,缓解产妇负面情绪,同时有利于增加产妇产后泌乳量,提高产妇母乳喂养率,优化婴幼儿的喂养方式,具有临床应用价值,值得推广。

Objective To study the effect of cold spray combined with dredging meridians and collaterals Tuina on early postpartum breast distension and breastfeeding and provide clinical guidance for the recovery of parturients.Methods From January 2019 to September 2019,90 parturients with breast distention and pain were selected as the study objects.According to the method of random number table,they were divided into study group and control group,45 cases in each group.The control group was treated with dredging meridians and collaterals Tuina.The study group was treated with cold spray of traditional Chinese medicine on the basis of the control group.The degree of postpartum breast pain,breast hardness,breast filling degree,the time and amount of lactation and the way of postpartum breastfeeding were measured and recorded.Results After treatment,the degrees of breast pain,breast hardness and breast filling in the study group were significantly better than those in the control group(P<0.05).The time of lactation in the study group was(16.04±4.89)h,which was significantly shorter than that in the control group[(23.07±5.77)h](P<0.05).After treatment,the lactation of the two groups were significantly increased(P<0.05),and 24,48 and 72 hours after treatment,the lactation of the study group was significantly higher than that of the control group.The difference was statistically significant(P<0.05).After treatment,the breastfeeding rate of the study group was 80.0%(36/45),and that of the control group was 60.0%(27/45).The difference was statistically significant(P<0.05).Conclusion The early intervention of cold spray of traditional Chinese medicine combined with dredging meridians and collaterals Tuina is beneficial to relieve the pain of postpartum breast distention and pain,improve the negative emotions of the parturients,increase the amount of postpartum milk production,increase the rate of breastfeeding,and optimize the way of infant feeding.It has clinical application value and is worthy of popularizing.
