自拟消炎汤佐治重度牙周炎效果及对龈沟液黏附因子、炎症因子和MMP-8、MMP-13水平的影响

Effects of Self-Made Xiaoyan Decoction(自拟消炎汤)in Treatment of Severe Periodontitis and Influence on Levels of Gingival Crevicular Fluid Adhesion Factors,Inflammatory Factors,MMP-8 and MMP-13

目的探讨自拟消炎汤佐治重度牙周炎效果及对龈沟液黏附因子、炎症因子和基质金属蛋白酶-8(MMP-8)、MMP-13水平的影响。方法选择本院2016年3月—2018年4月收治的60例重度牙周炎患者,随机数字表法分为研究组和对照组各30例。对照组给予常规西药治疗,研究组在其基础上给予自拟消炎汤治疗。比较两组患者治疗前后龈沟液黏附因子、炎症因子和MMP-8、MMP-13水平,并观察两组治疗效果。结果治疗后,观察组总有效率为93.33%(28/30),明显高于对照组的73.33%(22/30)(P<0.05)。研究组患者黏附因子[可溶性细胞间黏附分子-1(sICAM-1)、可溶性血管黏附分子-1(sVCAM-1)]、炎症因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)]、MMP-8和MMP-13水平均明显低于对照组(P<0.05)。结论自拟消炎汤佐治重度牙周炎疗效显著,可有效缓解患者牙周组织炎症反应,改善临床症状。

Objective To investigate the effects of Self-made Xiaoyan Decoction(自拟消炎汤)in the treatment of severe periodontitis and the influence on levels of gingival crevicular fluid adhesion factors,inflammatory factors,matrix metalloproteinase-8(MMP-8)and MMP-13.Methods Sixty patients with severe periodontitis admitted to the hospital between March 2016 and April 2018 were selected.They were divided into the study group and the control group by the random number table method with 30 cases in each group.The control group was treated with conventional western medicine,and on the basis,the study group treated with Self-made Xiaoyan Decoction.The levels of gingival crevicular fluid adhesion factors,inflammatory factors,MMP-8 and MMP-13 were compared between the two groups before and after treatment,and the therapeutic effects were observed.Results After treatment,the total response rate of the observation group was significantly higher than that of the control group(93.33%(28/30)vs 73.33%(22/30))(P<0.05).The levels of adhesion factors[soluble intercellular adhesion molecule-1(sICAM-1),soluble vascular adhesion molecule-1(sVCAM-1)],inflammatory factors[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),interleukin-8(IL-8)],MMP-8 and MMP-13 in the study group were significantly lower than those in the control group(P<0.05).Conclusion Self-made Xiaoyan Decoction is markedly effective in the treatment of severe periodontitis.It can effectively alleviate periodontal tissue inflammation and improve clinical symptoms.
