疏肝解郁胶囊联合个性化愉悦元素积极刺激干预对脑卒中后抑郁患者5-HT、NE、DA的影响

Effects of ShuganJieyu Capsule(疏肝解郁胶囊)and Personalized Pleasure Elements Intervention on 5-HT,NE and DA in Patients with Post-stroke Depression

目的探讨疏肝解郁胶囊联合个性化愉悦元素积极刺激干预对脑卒中后抑郁患者血清5-羟色胺(5-HT)、去甲肾上腺素(NE)、多巴胺(DA)水平的影响。方法选取2017年1月—2019年3月医院收治的64例脑卒中后抑郁患者作为研究对象。采用随机数字表法将其分为对照组(n=32)和研究组(n=32),其中对照组给予西医综合治疗联合个性化愉悦元素积极刺激干预,研究组在此基础上给予疏肝解郁胶囊口服治疗,两组均连续治疗3个月。比较两组治疗前、治疗1、3个月后的抑郁情况;对比分析两组治疗前、治疗3个月后的生活质量及血清细胞因子水平;统计两组治疗期间的不良反应发生情况。结果与治疗前比,治疗后1、3个月,两组汉密尔顿抑郁量自评表(HAMD)评分呈逐渐降低趋势,且治疗后1、3个月研究组均显著低于对照组(P<0.05)。与治疗前比,治疗后3个月,两组生活质量各维度评分均显著升高,且研究组均显著高于对照组(P<0.05);两组血清白细胞介素-6(IL-6)、C反应蛋白(CRP)、同型半胱氨酸(Hcy)水平均显著降低,且研究组均显著低于对照组(P<0.05);两组血清5-HT、NE、DA水平均显著升高,且研究组均显著高于对照组(P<0.05)。研究组的不良反应发生率为9.38%(3/32),显著低于对照组的不良反应发生率25.00%(8/32)(P<0.05)。结论疏肝解郁胶囊联合个性化愉悦元素积极刺激干预治疗脑卒中后抑郁,能够显著降低患者血清5-HT、NE、DA水平,减轻患者抑郁程度,并能提高患者生活质量,且具有较高安全性,值得推广。

Objective To investigate the effects of Shugan Jieyu Capsule(疏肝解郁胶囊)combined with personalized pleasure elements intervention on the levels of serum 5-hydroxytryptamine(5-HT),norepinephrine(NE)and dopamine(DA)in patients with post-stroke depression.Methods Sixty-four patients with post-stroke depression treated in our hospital between January 2017 and March 2019 were selected as the research subjects.The random number table method was used to divide them into the control group(n=32)and the research group(n=32).The control group was given comprehensive western medicine combined with personalized pleasure elementsand the research group was given Shugan Jieyu Capsule for oral treatment,and the two groups were treated continuously for 3 months.Depression was compared before and after treatment for 1 and 3 months in the two groups.The quality of life and serum cytokine levels in the two groups were compared before and after treatment for 3 monthsand the incidence of adverse reactions during the treatment period was calculated.Results Compared with those before treatment,the scores ofHamilton depression scale(HAMD)in the two groups showed a gradual decrease trend at 1 and 3 months after treatment(P<0.05),and the research group's was obviously lower than that of the control group at 1 and 3 months after treatment.The difference was statistically significant(P<0.05).Compared with those before treatment,3 months after treatment,the scores of quality of life were significantly increased in the two groups,and the research group's was obviously higher than that of the control group.The difference was statistically significant(P<0.05).The levels of serum interleukin-6(IL-6),C-reactive protein(CRP)and homocysteine(Hcy)were significantly reduced in the two groups,and the research group'swere obviously lower than those of the control group.The difference was statistically significant(P<0.05).The levels of serum 5-HT,NE and DA were significantly increased and the research group's were obviously higher than those of the control group.The difference was statistically significant(P<0.05).The incidence of adverse reactions in the research group was 9.38%(3/32),which was obviously lower than that in the control group(25.00%,8/32).The difference was statistically significant(P<0.05).Conclusion Shugan Jieyu Capsule combined with personalized pleasure elements intervention can significantly reduce the levels of serum 5-HT,NE and DA,reduce the degree of depressionand improve the quality of life of patients with post-stroke depression.It has high safety and is worthy of promoting.
