高分辨率超声在甲状腺乳头状癌诊断中的应用价值分析 被引量:3

摘要 目的分析高分辨率超声在甲状腺乳头状癌诊断中的应用价值。方法选取2018年10月至2019年10月我院收治的120例高度疑似甲状腺乳头状癌患者,均接受高分辨率超声检查。以病理检查结果作为“金标准”,评估高分辨率超声在甲状腺乳头状癌诊断中的灵敏度、特异度及准确度,并进一步分析高分辨率超声诊断甲状腺乳头状癌的声像特征。结果高分辨率超声诊断甲状腺乳头状癌的准确度为94.17%,灵敏度为95.10%,特异度为88.89%,与病理检查结果一致性较好(κ=0.786)。甲状腺乳头状癌病灶超声主要表现为低回声,病灶内多见微钙化,病灶边缘多模糊不清,病灶内部血流信号丰富,多为高阻力血流病灶。结论高分辨率超声能够清晰显示甲状腺乳头状癌病灶内部回声、血流信号、钙化特征及病灶边缘等信息,诊断准确度较高,可作为临床鉴别甲状腺乳头状癌的有效手段。 Objective To analyze the application value of high-resolution ultrasound in the diagnosis of papillary thyroid carcinoma.Methods 120 patients with highly suspected papillary thyroid carcinoma admitted to our hospital from October 2018 to October 2019 were selected and given high-resolution ultrasound.Taking pathological examination results as the gold standard,the sensitivity,specificity and accuracy of high-resolution ultrasound in the diagnosis of papillary thyroid carcinoma were evaluated,and the ultrasonic image characteristics of high-resolution ultrasound in the diagnosis of papillary thyroid carcinoma were further analyzed.Results The accuracy,sensitivity and specificity of high-resolution ultrasound in the diagnosis of papillary thyroid carcinoma were respectively 94.17%,95.10%and 88.89%,which had better consistency with the pathological examination results(κ=0.786).Ultrasound of papillary thyroid carcinoma lesions was mainly hypoechoic,and microcalcification was more common in the lesion,the edges of lesions were blurred,and the blood flow signals inside the lesions were abundant,mostly with high resistance blood flow lesions.Conclusions High-resolution ultrasound can clearly display information such as internal echo,blood flow signal,calcification characteristics and the lesion edge in papillary thyroid carcinoma with higher diagnostic accuracy,which can be used as an effective method for clinical identification of papillary thyroid carcinoma.
