不同全子宫切除术对子宫肌瘤患者卵巢功能及性功能的影响

Impact of Different Total Hysterectomy on the Ovarian Function and Sexual Function in Patients with Uterine Fibroids

目的比较腹腔镜全子宫切除术和开腹全子宫切除术对子宫肌瘤患者卵巢功能及性功能的影响。方法选择2018年1月至2020年3月我院收治的子宫肌瘤患者86例,根据手术方式的不同分为两组各43例。A组行开腹全子宫切除术治疗,B组行腹腔镜全子宫切除术治疗。比较两组手术前后的卵巢功能及性功能。结果术后6个月,两组的E2水平均低于术前,FSH水平均高于术前(P<0.05);但两组间的E2、FSH水平比较,差异无统计学意义(P>0.05)。术后6个月,两组的FSFI量表各维度评分均高于术前,且B组的FSFI量表各维度评分均高于A组(P<0.05)。结论腹腔镜全子宫切除术与开腹全子宫切除术均会对子宫肌瘤患者的卵巢功能造成损伤,但腹腔镜手术对患者性功能的影响较小。

Objective To compare the impact of laparoscopic total hysterectomy and open total hysterectomy on the ovarian function and sexual function in patients with uterine fibroids.Methods 86 patients with uterine fibroids admitted to our hospital from January 2018 to March 2020 were selected and divided into two groups according to different surgical methods,with 43 patients in each group.Group A was treated with open total hysterectomy,and group B was treated with laparoscopic total hysterectomy.The ovarian function and sexual function before and after treatment were compared between two groups.Results At 6 months after surgery,the E2 levels of both groups were lower than those before surgery,and the FSH levels were higher than those before surgery(P<0.05),while no significant difference was found in the E2 and FSH levels between two groups(P>0.05).At 6 months after surgery,the scores of FSFI scale dimensions in both groups were higher than those before surgery,and the scores of FSFI scale dimensions in group B were higher than those in group A(P<0.05).Conclusions Both laparoscopic total hysterectomy and open total hysterectomy will damage the ovarian function of patients with uterine fibroids,but laparoscopic surgery has less impact on the sexual function of patients.
