面神经电图和瞬目反射检测在电针治疗急性面神经麻痹中的应用

The Application of Electroneurogam and Blink Reflex Detection in Electro Acupuncture Therapy in the Treatment of Facioplegia

目的探讨面神经电图(Electro-neurogram,ENoG)和瞬目反射(Blink reflex,BR)检测在电针治疗急性面神经麻痹中的应用效果。方法选择2016年7月—2019年3月就诊于大连大学附属新华医院的80例急性面神经麻痹患者,按随机数字表法分为电针组与对照组,各40例。对照组采用常规西医治疗,电针组在对照组治疗方案基础上加电针治疗,2组均治疗20 d。患者于10、20 d后,治疗前后分别进行患侧面神经ENoG和BR检测。比较2组的临床疗效、治疗前后患侧的BR潜伏期、眼轮匝肌和口轮匝肌复合肌肉动作电位(Compound muscle action potential,CMAP)潜伏期和波幅及动作电位(Action potential,AP)潜伏期和波幅。结果电针组的治疗总有效率为90.0%(36/40),高于对照组的82.5%(33/40)(P<0.05);电针组治疗后的R1波、R2波、R2'波均短于对照组(P<0.05);电针组治疗后的CMAP潜伏期短于对照组,波幅大于对照组(P<0.05);电针组治疗后的AP波幅大于对照组(P<0.05)。结论电针治疗急性面神经麻痹,能提高患者的临床疗效,ENoG和BR检测可以作为急性面神经麻痹诊断及疗效评价手段之一。

Objective To discuss the curative effective of electro acupuncture therapy in the treatment of facioplegia through electroneurogam(ENoG)and blink reflex(BR)detection.Methods Eighty patients with acute facial nerve palsy who were admitted to Xinhua Hospital Affiliated to Dalian University from July 2016 to March 2019 were selected were selected and randomly divided into electro acupuncture group and control group,with 40 patients in each group.Both of them were treated for 20d.After 10d and 20d of treatment,ENoG and BR detection on the facial nerve were conducted before and after treatment.Clinical efficacy,BR latency,compound muscle action potential(CMAP)latency and action potential(AP)latency of orbicularis oculi and orbicularis oris were compared between the two groups before and after treatment.Results The total treatment rate of electro acupuncture group was 90.0%(36/40),which was higher than that of control group(82.5%)(33/40)(P<0.05).After treatment,R1 wave,R2 wave and R2'wave of electro acupuncture group were shorter than those of control group(P<0.05).The latent period of CMAP in electro acupuncture group was shorter than that in control group,and the amplitude of CMAP was larger than that in control group(P<0.05).AP amplitude of electro acupuncture group after treatment was higher than that of control group(P<0.05).Conclusion Electro acupuncture therapy is effective to treat facioplegia.ENoG and BR can be used as one of means to diagnose facioplegia and evaluate its curative effect.
