腹腔镜根治性全胃切除术治疗胃上部癌的近期效果及安全性分析

Analysis of the short-term effect and safety of laparoscopic radical total gastrectomy in the treatment of upper gastric cancer

目的:分析腹腔镜根治性全胃切除术治疗胃上部癌的近期效果及安全性。方法:2019年1月-2020年1月收治胃上部癌患者38例,随机分为两组,各19例。对照组给予传统开腹根治性全胃切除术治疗;观察组给予腹腔镜根治性全胃切除术治疗。比较两组手术效果。结果:观察组淋巴结清扫数量多于对照组,术中出血量少于对照组,术后24 h疼痛(VAS)评分低于对照组,差异有统计学意义(P<0.05);观察组排气恢复时间、自主下床活动时间、进食流质食物时间及住院时间均短于对照组,差异有统计学意义(P<0.05);观察组术后并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:采取腹腔镜根治性全胃切除术治疗胃上部癌的近期疗效明显,术后并发症发生率低。

Objective:To analyze the short-term effect and safety of laparoscopic radical total gastrectomy in the treatment of upper gastric cancer.Methods:From January 2019 to January 2020,38 patients with upper gastric cancer were selected,they were randomly divided into the two groups with 19 cases in each group.The control group was treated with traditional open radical gastrectomy.The observation group was treated with laparoscopic radical total gastrectomy.We compared the surgical effects of the two groups.Results:The number of lymph node dissections in the observation group was more than that of the control group,and the amount of intraoperative blood loss was less than that of the control group,the pain(VAS)score at 24 h after the operation was lower than that of the control group,the differences were statistically significant(P<0.05).The recovery time of exhaust,the time of getting out of bed,the time of eating liquid food and the length of hospital stay in the observation group were shorter than those in the control group,the difference was statistically significant(P<0.05).The postoperative complication rate of the observation group was lower than that of the control group,the difference was statistically significant(P<0.05).Conclusion:Laparoscopic radical total gastrectomy has obvious short-term curative effect in the treatment of upper gastric cancer,and the incidence of postoperative complications is low.
