牙科焦虑对牙周膜龈美学手术后疼痛的影响

The effects of dental anxiety on the postoperative pain of patients after periodontal plastic surgery

目的:评估牙科焦虑和恐惧水平以及相关影响因素对患者牙周膜龈手术术后疼痛水平的影响。方法:纳入152例患者的相关信息,完成焦虑评估量表,反馈术后疼痛水平,对结果进行统计分析。结果:女性的SAI评分比男性高5.36分(P=0.004),SAI评分随着术前预期疼痛程度的增加而增加(β=1.29,P<0.001)。女性性别(β=2.39,P=0.045),不良口腔手术经历(β=7.08,P<0.001)和较高的预期疼痛水平(β=0.83,P=0.001)与IDAF-4C评分增加有关。较高的IDAF-4C评分(β=0.07,P=0.009)、较长的手术时间(β=0.06,P<0.001)与较高术后疼痛水平显著相关。较高的教育水平与较低的术后疼痛有关(β=-0.84,P=0.035)。结论:术前患者对治疗目的、方法及意义的了解程度以及来自医护人员的心理疏导,可以减轻患者的焦虑,增加患者手术的依从性,减少牙周膜龈美学手术患者的术后疼痛。

Objective:To evaluate the influence of dental anxiety and related factors on postoperative pain level of patients after periodontal plastic surgery.Methods:The demographic information,anxiety assessment scale and feedbacks of postoperative pain level of 152 patients,underwent periodontal plastic surgery,were investigated,the results were statistically analyzed.Results:The SAI score of women was 5.36 points higher than that of men(P=0.004).The SAI score increased with the increase of preoperative expected pain level(β=1.29,P<0.001).Female gender(β=2.39,P=0.045),unpleasant oral surgery experience(β=7.08,P<0.001)and higher expected pain level(β=0.83,P=0.001)were associated with increased IDAF-4C score.Higher IDAF-4C score(β=0.07,P=0.009)and longer operation time(β=0.06,P<0.001)were significantly associated with higher postoperative pain level.Higher education level was associated with lower postoperative pain level(β=-0.84,P=0.035).Conclusion:Understanding of the purpose,significance and specific methods of the operation from the patients and the psychological counseling from the medical staff may help the patients to increase compliance,reduce the pain,decrease the anxity and postoperative pain for periodontal plastic surgery.
