摘要
目的本研究旨在探究低剂量多层螺旋CT灌注成像在诊断良恶性甲状腺结节中的应用效果情况。方法本实验选取2019年9月—2020年7月于医院诊疗的患有甲状腺结节且最终进行手术的患者共计120例,所有患者均进行CT灌注扫描及术后病理检查,根据患者术后病理良恶性将入组患者分成良性组及恶性组,其中良性组78例、恶性组42例。对两组患者CT灌注成像下的血流量(blood flow,BF)、出血容量(bleed volume,BV)、平均通过时间(mean transit time,MTT)、达峰时间(time to peak,TTP)及影像学表现进行对比分析。结果良性组患者MTT、TTP均高于恶性组,BV、BF均低于恶性组,以上差异均具有统计学意义(P<0.05);良性组患者结节边缘清晰、形态规则及条片状钙化占比明显高于恶性组,差异具有统计学意义(P<0.05)。结论研究结果显示低剂量螺旋CT灌注成像对于甲状腺结节良恶性鉴别中有着较高的指导价值。
Objective To explore the application effect of low-dose multi-slice spiral CT perfusion imaging in the diagnosis of benign and malignant thyroid nodules.Methods Selected 120 patients with thyroid nodules from September 2019 to July 2020 for surgery.All patients underwent CT perfusion scan and postoperative pathological examination.According to the patients postoperative pathological benign and malignant patients were divided into the benign group and the malignant group,including 78 cases in benign group and 42 in malignant group.The blood flow(BF),bleed volume(BV),mean transit time(MTT),time to peak(TTP)and imaging performance under CT perfusion imaging of the two groups of patients were compared and analyzed.Results The MTT and TTP of the benign group were higher than those of the malignant group,and the BV and BF were lower than those of the malignant group.The above differences were statistically significant(P<0.05);the benign group had clear nodules,regular morphology and strips proportion of calcification was significantly higher than that of the malignant group,and the difference was statistically significant(P<0.05).Conclusion The research results show that low-dose spiral CT perfusion imaging has a high guiding value in the differentiation of benign and malignant thyroid nodules.
