摘要
目的分析脑胶质瘤相关癫痫病人量化脑电参数的特点。方法回顾性分析45例脑胶质瘤病人的临床及术前脑电资料。筛选无明显伪差的段落,提取包络值、总功率、绝对频段能量、相对频段能量、α/δ波比率(alpha-delta ratio,ADR)以及光谱边界等量化脑电参数,分别对无癫痫发作史脑胶质瘤病人的以上量化脑电参数进行统计对比。结果在颞叶胶质瘤病人中,与无癫痫发作史比较,在F7-F8导联中,有癫痫发作史病人包络值、总功率和δ、θ频段绝对能量均显著升高(P<0.05),而光谱边界显著降低(P=0.041);T3-T4导联中,有癫痫发作史病人总功率、δ频段绝对和相对能量均显著增高(P<0.05),而β频段相对能量和光谱边界均显著降低(P<0.05);T5-T6导联中,有癫痫发作史病人δ频段绝对和相对能量均显著升高(P<0.05),ADR、β频段相对能量和光谱边界均显著降低(P<0.05)。结论颞叶胶质瘤相关癫痫病人脑电发作间期在定量脑电图存在特征性改变,对致痫灶具有定位意义,有益于完善术中策略。
Objective To explore the characteristics of quantitative electroencephalogram(EEG)parameters in patients with glioma-related epilepsy(GRE).Method Clinical and preoperative EEG data of 45 glioma patients were analyzed retrospectively.The specific periods without obvious artifacts were selected to extract the parameters including the envelope value,total power,absolute band power,relative band power,alpha-delta ratio(ADR)and spectral boundary.The above quantitative parameters of patients with or without seizure were statistically compared.Results Among patients with temporal lobe glioma,compared with patients without seizure,those patients with seizure showed significant increases in the envelope value,total power,and absolute power of theδandθbands(P<0.05),and the spectral boundary in the F7-F8 lead was significantly reduced(P=0.041).In the T3-T4 lead,the total power,the absolute and the relative power ofδ-band were significantly increased(P<0.05),while the relativeβ-band power and spectral boundary were significantly reduced(P<0.05).In the T5-T6 lead,the absolute and relative powers of theδ-band were significantly increased(P<0.05),while ADR,the relativeβ-band power and the spectral boundary were significantly reduced(P<0.05).Conclusion The specific alteration exist in quantitative EEG ofinterictal period in patients with temporal lobe GRE,which is of significance for the location of epileptic foci and is beneficial to the improvement of intraoperative surgical strategies.
